He did everything medicine told him to do… and it nearly cost him his life. In this episode of the Out of the System Podcast, Dr. Joshua Schacter joins us to share how burnout, loss of autonomy, and a broken insurance model pushed him to rebuild medicine around time, integrity, mindset, and truly patient-centered care.

He reached the top of his profession… and realized something was deeply wrong. In this episode of the Out of the System Podcast, Dr. Joshua Schachter shares how years inside hospital-based medicine quietly stripped autonomy, connection, and meaning from his work.

We explore what happened when he stepped outside the system and began rebuilding care around time, mindset, and patient-first decisions. Our conversation also delves into why burnout isn’t a personal failure, how inherited training shapes physician behavior long after residency, and what becomes possible when doctors reclaim agency over how they practice and how they heal.

Dr. Joshua Schacter is an orthopedic surgeon and the founder of Pinnacle Integrative Orthopedics and Sports Medicine. After more than a decade in a hospital-employed role, performing thousands of surgeries and holding multiple leadership positions, he reached a breaking point with burnout, loss of autonomy, and volume-driven care.

Today, Dr. Schacter practices integrative, cash-based orthopedics focused on time, mindset, functional medicine, and patient-centered healing. He has built a practice rooted in integrity, joy, and sustainable outcomes for both patients and physicians.
