The biggest lie in medicine may be the one doctors were trained never to question. In this solo episode of the Out of the System Podcast, Matthew Gillogly reads from his book Out of the System and argues that many providers have been conditioned by medical training, debt, fear, and overwork to become compliant employees inside a system that profits from their license while stripping away autonomy.

He walks through what he calls the “white coat indoctrination,” challenging the beliefs that long hours create success, debt is just part of the journey, and volume-based care is somehow noble. He also points to insurance-driven protocols, rushed patient visits, RVU-based compensation, and cash-pay pricing examples as evidence that the current model rewards bureaucracy over care and keeps providers stuck in burnout and dependence.
