spend some time on because the situation Pat is navigating
is one that affects millions of Americans. So Pat writes
in from Farmington, New Mexico, and Pat writes, I'm on Eloquist.
That's a blood thinner, and I also have a FIP.

Because of the Eloquist, I have to be careful about

but I don't know what is safe and what could
interfere with my medication. Any help would be greatly appreciated, again,

you for a question and for being thoughtful enough enough
to ask it before acting. That instinct alone puts you

ahead of a lot of people who are in the
exact situation. So let me give you a framework that
I think will serve you well. Because this is a
two part conversation. The first part is about Eloquist, specifically
what it interacts with in what it does not. The
second part is about a FIB and the very real,
evidence supported role that integrated medicine plays in managing it.

Eloquist, the generic name is picks A van, is a
direct oral anti coagulant or DOAC. It works by an
hippiting factor x A in the clotting cascade. One of
the significant advantages of Eloquist over older blood thinners like

wharfin is that it does not have the same sensitive
to vitamin K in your diet. That means the leafy
green conversation that wharf and patients they dreaded that we're
talking about the kale, spinach, broccoli. It's far less of
an issue for you need to avoid these foods. In fact,

I would encourage that you do eat them. They have
lots of anti inflammatory qualities and obviously we know inflammation
is one of the key drivers to that a fit
that arterial fibrillation. What you do need to be careful
about with Eloquist falls into two categories, and we're talking
about supplements that then the blood further in substances that

affect how your liver metabolizes the drug. On the supplement side,
high dose fish oil above two to three grams daily
can add an additive blood thinning effect at low doses
with physician monitoring. I want to add Omega three's are
actually beneficial for cardiac rhythm and inflammation, but the dosage

does matter. And now we're talking about if we're talking
about other supplements like nanocinase in sarah peptase. These are
popular natural clot busting end zone times, they should be
avoided while on eloquists. The same goes for high dose
vitamin E, gink o bloba, and concentrated garlic supplements at

those huge therapeutic doses. Now, these are not necessarily dangerous foods.
They are dangerous at huge concentrations when combined with anti coagulants.
So if you've got questions about some of these supplements,
feel free to reach out further. Now, grapefruit in Saint

John's Wart both affect the liver enzymes that process eloquists,
and grapefruit can increase those drug levels. Saint John's Wart
can lower them, so avoid both of those. Now here's
the part I want you to hold on to pat
because this is where integrated medicine has a genuinely important

role in the management of a FIP. Magnesium glycinate is
one of the most underutilized tools in cardiac care. Magnesium
plays a direct role in electrical conduction in the heart,
and deficiency is extremely common in people with a fit,
so research supports magnesium supplementation for reducing the frequency of

arterial fibrillation episodes. It does not significantly interact with eloquists,
So we're talking about dosage of two hundred to four
hundred milligrams of magnesium glycinate daily and taken into evening.
It's something worth discussing with your cardiologists. And coenzyme q
ten is another one. Coenzyme q ten supports that energy

production in cardiac muscle cells and has demonstrated anti arrhythmic
properties in clinical research. It does not interact with eloquists
in any clinically significant way and is generally well tolerated.
So we're talking about dosage of one hundred to two
hundred milligrams daily with a fat containing meal. That's pretty typical.

So beyond supplements, we're talking stress reduction. Vaguel nerve in
the autonomic nervous system are deeply involved in triggering a

I'm calling from Corville, Iowa, and I love the Doctor
Bob show. He had a couple of questions where maybe
if somebody could call me back. First thing, my wife
has been dealing with she's in her sixties now, and
she's been dealing with hot flashes that have just not

gone away over the last few years. And so I
wonder there's something that you might suggest for her hot flashes.
But she's into holistic medicine. She's doing all this stuff
with homeopathy. She's really working hard to figure it out,
but so far it's been a struggle.

So I don't know. There might be something there as
you could suggest. But also the toothpaste, Doctor Bob was
talking about a toothpaste that I should buy maybe at
a co op store or a health smart store or something.
What was that toothpaste that he's told me to get.
I just can't recall the name of it. So again

And Rob he's got two questions, one for his wife,
who has been battling persistent hot flashes in her sixties
and is already pursuing a holistic path. And the second
question is about the toothpaste that we've been recommending on

the show. So Rob, I've got answers for you, So
let's go ahead and get into it. So Rob, first,
your wife is clearly doing the right things. The fact
that she's already engaged with holistic medicine, working with homeopathy,
and actively researching her own health puts her in a
position where we're not starting from scratch. We're building on

(15:33):
a foundation, and I feel like that matters. So we're
talking about hot flashes that persist well into her sixties,
and that's not unusual, and they are not simply a nuisance.
There sometimes a signal the body is communicating this hormonal
shift that is still active. In the intensity and the duration,

(15:56):
it varies enormously from woman to woman. The conventional response
is hormone replacement therapy, and for some women that is
absolutely appropriate, But for a woman that's already invested in
that natural approach, there is a meaningful toolkit that's worth exploring,
and that The research behind several of these options is

(16:19):
legitimate and it's growing. So let's start with black cohosh
and as co ho sh. This is the most clinically
studied herbal intervention for menopausal hot flashes, and it has
earned its place in that conversation. Multiple randomized controlled trials
have demonstrated a reduction in both the frequency intensity of

(16:43):
hot flash episodes. It works through a pathway that interacts
with serotonin receptors rather than estrogen receptors, which is why
it is generally considered safe for women who have had
hormone sensitive cancers. Standard dosing is twenty milligrams of extract

(17:04):
twice a day and it typically takes about four to
eight weeks to reach that full effect, so that patience
is key. These These fido estrogens are another powerful category here.
We're talking about the soy isoflabnoids. They're found in whole
soy foods, are in the supplemental form, and they bind

(17:26):
weekly to estrogen receptors and can help modulate that hormonal
fluctuations that trigger those hot flashes. They we're talking red
clover as well. They work through a similar mechanism as
soy in have have shown particularly promising results in clinical
trials for women in their sixties with persistent symptoms. These

(17:49):
are are not the same as synthetic estrogen. These are
plant based compounds that gently nudge that system in the
right direction. So both options, as far as the red
clover and soy are worth exploring for your wife. Maca

(18:10):
root and that's spelled maca maca root is one I
find myself recommending more and more. It does not act
as an estrogen mimic. It works on this hypothalamic pituitary
axis and it's supporting the body's own hormonal communication system.

(18:33):
So research is showing it reduces the severity of menopausal
symptoms including hot flashes, mood disruption, and sleep interference. And
so maca at one and a half to three grams
daily is what the research is supporting. Let's move on
to sage and specifically sage leaf extract. It has a

(18:57):
long history of use for reducing night sweats and hot flashes,
and there is modern clinical data that supports this. It
appears to act as a mild antiperspirant at an ontonomic level,
and that's worth adding to this protocol. And also tell
your wife to look at magnesium glyconate in the evening

(19:19):
about three hundred to four hundred milligrams before bed. It
will not stop a hot flash, but it significantly improves
the sleep disruption that hot flashes cause, and a better
sleep improves everything dawnstream. Lastly, if she has not explored acupuncture,
it is one of the most researched non pharmological interventions

(19:41):
for menopausal hot flash, and a course of treatment here
looks like over six to eight weeks with a licensed
acupuncturist trained in women's health is worth that investment. So
Rob just got those great instincts, the combination of approaches,
discussing those with her practitioners, it gives her real evidence

not sure if this is all just a guessing game.
I'm actually looking for whether there is some specific steps
of how to go about finding help rather than just

blindly following a number for someone who has professed to
being a wellness guru who blindly sends you off into
a supplement company, which I'm sure they are getting a

kickback for every supplement that you buy. I just I'm
just lost and guessing that that everyone is just guessing.
I mean, I just don't understand where you start. Is
it genetic testing? I'm not sure if the science is

more reliable than going to a wellness doctor, spending five
hundred dollars and appointments and then having her just roll
the dice about which supplements are actually good for you.

is not paranoia, it's pattern recognition that the wellness industry
does have a money problem, and your instinct to question

is not only valid, it's one of those important tools
that you have right now. So let let me give
you a real roadmap and not a philosophy or a
sales pitch that you may have had. We're going to
give you a practical steps that will help you cut
through some of that noise and fine care that actually
deserves your trust. So step one, let's start with the

foundational labs before you spend any money on a functional
medicine practitioner or a single supplement. So go to your
primary care physician and ask for a comprehensive baseline panel.
This is exactly what you want. A complete metabolic panel,
a full LIPPID panel, a complete blood count, a thyroid

Next step, before your first appointment with anyone, ask the
right questions. Ask them, what does your diagnosis process look
like before you make recommendations. If the answer is not
grounded in testing, a comprehensive intake history and clinical reasoning,

on the first visit without ordering lab tests is guessing.
You already know what guessing looks like. You described it
