Here's How To Register For The COVID-19 Vaccine In Phoenix

Maricopa County has officially reached Phase 1B in the process of distributing the COVID-19 vaccine to residents.

Registration for this round of distribution began on Monday for those receiving the vaccine at State Farm Stadium, which is a 24/7 facility.

To receive a vaccination, individuals must have an appointment as there will be no walk-ins allowed.

Here's who is currently eligible for the vaccine:

  • K-12 school staff and child care workers
  • Law enforcement/protective services including sworn officers and government-employed security officers
  • Adults age 75 and older
  • People who qualified for Phase 1A but were unable to get the vaccine

You can also contact ADHS by phone (602-542-1000 or 844-542-8201) or email (podvaccine-triage@azdhs.gov).

Once you have created a login and password, you will have access to the site appointment system that will give you instructions to get started. This will also help to determine if you are eligible for Phase 1B. You will be asked a series of questions, and then be required to enter your insurance information. Insurance is not a requirement for the vaccine.

Once you arrive to your appointment, you will need to have the following things ready to be shown:

  • appointment confirmation email
  • form of ID that will confirm your Phase 1B employment such as paystubs, a drivers license to confirm your age, etc.

The vaccine is free, but it may be wise to bring along your insurance card just in case.
