Why ADHD Often Goes Undiagnosed in Women


Many women with ADHD don’t realize they have it until adulthood. Sometimes, they go undiagnosed until their 30s or 40s. Why? Because ADHD in women rarely fits the loud, impulsive stereotype shown in boys.

Girls are typically socialized to be agreeable. So when a girl with ADHD shows symptoms, fidgets, daydreams, or zones out, she learns to hide it. Instead of acting out, she internalizes the struggle. This girl might become a perfectionist and a people pleaser.

Recent research from the Journal of Attention Disorders found that adult women diagnosed with ADHD later in life reported a significant drop in self-blame and emotional distress after receiving confirmation.


Common Misdiagnoses and Overlapping Conditions


Because ADHD in women often presents internally instead of hyperactivity, it’s frequently mistaken for other mental health conditions.

Here are some common misdiagnoses:

  • Anxiety disorders. Anxiety can be secondary in women with ADHD, driven by missed deadlines, overwhelm, or sensory overload.

  • Depression. Emotional exhaustion and low motivation can mirror depressive symptoms.

  • Borderline personality disorder (BPD). Emotional reactivity, impulsivity, and fear of abandonment are often linked to BPD. Still, in some women, these traits stem from untreated ADHD combined with rejection sensitivity.

  • Complex PTSD. ADHD and trauma can both lead to emotional dysregulation, memory lapses, and difficulty staying present.

  • Bipolar II. Fluctuations in energy, focus, and emotional highs or lows can sometimes resemble bipolar spectrum disorders.<
