The program has expanded beyond acute care to include post-acute services traditionally provided in skilled nursing facilities, creating a comprehensive home care ecosystem that meets patients where they are.

From remarkably simple technology to the personal touch of in-home visits, Hospital Care at Home is providing patients with dramatically better outcomes: zero hospital-acquired infections, minimal delirium, less physical deconditioning, and readmission rates below 10%.
