Low ferritin is specific for iron deficiency, but high iron can be many things, including iron overload, infections, inflammation, and cancer! The stepwise evaluation of high ferritin includes transferrin saturation and other iron studies, followed by an evaluation for autoimmune diseases, cancers, and MOST COMMONLY, metabolic syndrome, which includes type 2 diabetes, obesity, high blood pressure, and abnormal cholesterol.
