Well, I have completed DBTeducation with Dr Alan Frazetti

and DBT is also part of mydoctoral project, research and
development.
So I speak about the theoriesas they would intersect with how
we, as nurses and frontlineresponders, move through the
world.
And, as a nurse who's livedthrough trauma, burnout and a
whole ass career on the edge,I've learned how to use these
skills as a practitioner, ateacher and a fellow human being

trying to survive the system.
So what we're talking abouttoday, it's not a therapy
session.
It's nurse to nurse, wisdom,real tools, real talk and
radical compassion for myself.
So let's talk about what thehell happened this week.
This week, emotional laborshowed up with a baseball bat.

(01:42):
I was overwhelmed behind anhour late for my own students.
And you know what?
What I didn't do.
I didn't lose my cool.
Why?
Because I stopped trying tomeet some bullshit internal
metric of perfection and justfocused on the next thing in
front of me.
And yeah, I could have spiraled, I could have gone like full

(02:05):
shame tornado, but I didn't.
The overwhelm is something that,especially with ADHD and being
neurodiverse, when I get to thepoint where I'm having to move
from one thing to the other andI'm still behind the curve,
meaning like there's no pause,there's no reflection, there's
no checking to see if noreflection, there's no, you know

(02:25):
, checking to see if yourexecutive functioning is even on
board that's when overwhelm forme becomes kind of like a tidal
wave.
So let me tell you whatactually helped Lowering the
engagement expectations that Ihad for myself.
As in, you don't have toimmediately respond to every
text like you're putting out a.
In, you don't have toimmediately respond to every
text like you're putting out afire.

(02:47):
You don't have to make planswith people or invest energy or
time with people, not becauseyou're being mean or excluding
them or being weird, but simplybecause you're conserving the
dopamine that you have for thefunctional necessity that you
need it for in that moment.
Once you've got your shittogether, relatively speaking,

(03:11):
and you're not in that moment ofoverwhelm, you can resume your
normal social activities andinteractions with friends and
engagements, if that's somethingthat works for you.
But lowering the engagementexpectations is a really, really
important factor in that thoseexpectations kind of hang in
front of you as if there's somekind of metric that you need to

(03:33):
reach while you're busy, liketrying to leap over hurdles in
the fastest 200 sprint of yourlife.
Newsflash, your girl's not asprinter.
The 800 was the fastest I everran.
I'm more of a distance person,so doing one task at a time,
even if I was late, we have thispropensity to want to crash

(03:57):
through tasks like a bull in aChina shop.
Whenever we get behind, lateand pressed for time and things
start stacking up against eachother without a moment to
breathe, we really try to graspeverything and throw all of it
out there and just hopesomething sticks.
All that does is add to theanxiety and the overwhelm in a

(04:19):
massive way, because then we'renot sure what fell through the
cracks, we're not sure if we didsomething, we're not sure how
many mistakes we made.
So just focusing on doing onetask at a time actually helps
you move through the tasks andachieve completion a hell of a
lot faster than our infamousneurodiverse multitasking does

(04:42):
in this situation of overwhelm.
Now don't get me wrong.
One of the reasons I was reallygood in the ER and the ICU is
because my brain is like thislittle hyper threaded thing up
there that occasionally reallysurprises me.
Another thing is showing up withhonesty instead of shame.
So this one we're going towe're going to talk about a
little bit more in depth in acouple minutes here, because

(05:03):
it's really really significantfor me Um, having rejection,
sensitive dysphoria as somebodywith ADHD, um, and a couple
other factors, this is a reallysignificant one for me.
But showing up with honestyinstead of shame actually keeps
you from compounding that and itreally keeps you from creating

(05:23):
this false narrative thatdepletes your dopamine far
faster than the situation ofoverwhelm does and ends up with
you feeling even moreunaccomplished and even more
behind and dejected.
And this is all this like fakenarrative.
Like girl, you're sitting inthe corner hurting your own
feelings.
Okay, just for real.

(05:45):
But we're going to talk aboutthat a little bit in a few more
minutes, a little bit later on.
Um, in regards to some basicskill sets, um, the last thing
was treating myself like a humanbeing instead of a productivity
bot.
So, recognizing if I need abreak, recognizing if I'm hungry
, recognizing, you know, just,inherent biological needs.

(06:08):
And again, what do we talkabout in this podcast?
Ad nauseum?
Intentionally creating space sothat you can be emotionally
aware, you can be self-aware,you can check in with yourself.
A lot of times when we get intooverwhelmed situations like
this whether it's on the floor,whether it's with our toddler,

(06:28):
whether it's, I don't know juststanding by yourself in the
middle of the parking lot, butwe can't get our shit together
that overwhelm really pushes usinto a realm of, okay, I have
got to be on it and I amchecking off boxes and I'm a
machine and I'm cranking thisout, and none of that holds any

(06:50):
space whatsoever for actuallybeing aware of yourself, your
surroundings and beingintentional with your actions.
So that's kind of the bulletpoints of what actually helped.
What saved me wasn't doing somemiracle of executive functioning
.
Let me tell you something.
When you, even if you'reneuro-normative, when you are in
situations of overwhelm likethat, I mean, come on, I was an

(07:12):
hour late for my own students,and we're not even going to talk
about why, because I hadseveral things, starting from
6.30 in the morning onward, thatwere back to back, slammed up
against each other, that wereback to back slammed up against
each other, one of which becauseI completely transposed the
times of one event on Friday andthen the event on Monday.
So, in absolute panic mode, Iwas already late running from

(07:36):
one event that I did.
Okay, on the first event Iwasn't initially late for, but
then that cascaded right intosomething I was 26 minutes late
for, ended up then being an hourlate for my students and it
just kept going from there.
I mean straight up, headlock,like I was about ready to fight.

(07:56):
But in retrospect, obviouslyit's not some miracle of
executive functioning, becausein situations like that that I
just described, our executivefunctioning is highly limited.
That fight or flight responsethat we get when the sympathetic
nervous system kicks in andstarts shunting blood all over

(08:18):
the place really does a dirty onexecutive functioning,
nevermind the neurotransmittersthat you're burning wildly while
you're scrambling for afoothold on your day or the
situation.
So your executive functioningexits stage left and if you're
neurodiverse, it might'vealready exited stage left before
you got up that morning.

(08:38):
Occasionally happens, so it's.
It's a it's a real struggle busLike it's.
That's not, um, that's not anexaggeration or hyperbole, but
again, it's not some miracle ofexecutive functioning, it's
distress tolerance.
Straight up, I dropped theperformance act and just
survived the moment.

(09:00):
Distress tolerance doesn't meanthat you go through the moment
or the situation and, you know,are clutching your pearls and
wearing your heels while doingit.
It means you just survived themoment and whatever that happens
to look like, I mean, hey, ifyou happen to be wearing a pearl
necklace and can wear heelsduring this, more power to you.
If you look like a wet, longhaired Persian cat by the end of

(09:25):
it, okay, also, you survivedthe moment, that's just fine.
So in DBT, this is kind of theart of riding the wave you don't
fight it, you don't fix it, youjust get through the damn thing
without making it worse.
That's the key aspect, thecoping mechanism that you employ

(09:47):
to get through the situation.
The point is to choose ahealthy one or the healthiest
one that you can choose in themoment, so that you're not
making the situation worse.
Fixing it may not be an option,or fixing it may be an option,
but you need executivefunctioning to do that.
And in order to get thatexecutive functioning, you got

(10:09):
to survive the moment, to giveyourself some space from it,
recoup and then address theproblem.
So let me walk you through howthis plays out in real life.
The first one is stop.
I literally said to myself girl, stop, you are not going to
think your way out of this panicspiral.
So I paused, breathed and askedwhat is the next thing I can

(10:34):
actually do?
What's the next thing?
Okay, I can show up at 826 andnot let the internal screaming
voice of panic be staring at the26 minutes of being late.
I can show up and beprofessional and participate and
be fully present, regardless ofhow being late was affecting me

(10:57):
.
So, basically, you stop, take astep back, observe and proceed
mindfully.
That mindfully part's reallyimportant, because the next one
is being one mindfully in themoment.
What the hell does that mean?
That means I didn't try to fixeverything.

(11:17):
I wasn't trying to catch up tosome imaginary schedule.
I chose one task.
I chose one task, just one, andI gave it everything I had in
that minute.
That's it.
Be one mindful in that moment.

(11:40):
Radical acceptance is the nextone.
This one hurts but it works.
This is one I struggle so hardwith as an eldest, grandchild,
firstborn daughter, latediagnosed ADHD, indigo kid.
I have to force myself toaccept that the nuts have left
the rave and there's no ducks,just trash pandas and a set of

(12:01):
scrubs.
So I had to say to myself okay,you're late, you're behind,
that's real.
Fighting it doesn't undo it.
But how you show up next, thatpart you still own to give
yourself a sense of control overthe situation.
Sometimes you have to radicallyaccept that it is a dumpster

(12:21):
fire and a dumpster fire yougleefully lit with your own
Molotov cocktail, howeverunintentionally.
You got to accept it, you gotto accept it.
And you have to put that hypervigilant fear of judgment and
rejection aside.
That's just not even part ofthe conversation.
You have to radically acceptwhat's real.

(12:42):
You have to look at what isreal and what is a narrative
going on in my head andspiraling, the stuff that's real
we accept.
Now how do I show up for this?
How do I show up for this andcarry on the self-respect effect
.
This is a big one.
I told my students the truth,not excuses, not fake apologies,

(13:05):
just honesty.
I'm human.
I had a hell of a rough starttoday, but I am fully here for
you now and guess what?
No one bailed, nobody judged me.
They respected me more becauseI led with integrity and not
performance.
They also understand that beingable to see somebody handling a

(13:26):
very similar stress loadperceptually to what they're
experiencing and how you show upfor it is really important.
So, yay, bonus me totallyunintentional falling downstairs
teaching moment.
But this is what I mean when Isay emotional exhaustion is not

(13:47):
a personality trait, it's astate, not an identity.
And if you treat it like astorm and not a personal failure
, there's that rejection,sensitive dysphoria thing again.
You can ride it out instead ofdrowning in it.
And it is really reallyimportant that we learn to
utilize these skills as we arenavigating everything from our

(14:10):
shift to our personalresponsibilities and
intersections and engagementswith others, because the type of
career and profession that wehave, we're already walking into
it at a higher stress level, ahigher demand and performance
expectation level than is normalor average because of the sheer

(14:34):
weight of responsibility on ourshoulders.
And guys, I'm not talking aboutjust the ICU or the ER or care
flight or first responders.
I am talking about every aspectin which you, as a licensed
professional, have theresponsibility of another human
being's welfare and life on yourshoulders.

(14:57):
That sounds like a bunch ofwords probably out of some
nursing textbook, but it's fact.
Every aspect of nursing.
We carry some facet, if not100%, of that burden of
responsibility on our shoulders,and that is a lot of stress.

(15:18):
That is a big mountain to bestanding in front of that you
have to get over to just evenstart the climb.
Okay, so that you know, radicalcompassion I mean, I was joking
when I said it was for myself,but it's really for all of us
and radical compassion issomething that you have to
develop by creating intentionalspace.

(15:40):
You can't breathe or look orobserve, or evaluate or even
stop if you haven't mastered theart of creating even just a
moment or two of intentionalspace.
In some of the earlier episodesI discuss how to use ritual a

(16:01):
practiced, habitual applicationof expectations, words, items,
behaviors to create intentionalspace.
And who am I kidding?
We all know I throw some magicin there too, but you have to be
able to do that in order toutilize any of these skills for

(16:21):
self-preservation, and that's100%.
What I am here to teach you isself-preservation how to protect
your peace and your sanity andput yourself first.
Now, on that note, we are goingto head for our usual musical
