Dr Robert J Troell M.D (00:00):
Who's an ideal candidate for rhinoplasty?
Actually anyone who wants animprovement of the cosmetic
appearance of their nose. In mythirty year history of doing
rhinoplasty surgery we havepeople who want a little tiny
tweak to the nasal tip and wehave people that we need
complete nasal reconstruction ormore difficultly which is
probably the most difficultthing in cosmetic or plastic

(00:21):
surgery is someone who's had aprevious nose job or rhinoplasty
and even more difficult has hadseveral of them. So when we look
at the nose from an aestheticstandpoint, it's what you as the
patient wants. Okay?
Well, I'll give myrecommendation what I think
aesthetically is the best. Oftenas an adjunctive procedure to

(00:42):
rhinoplasty, we do chin surgery. If you have a retruded chin,
micronathia it's called, if yourchin is too small, if we put a
chin implant at the same time wedo a rhinoplasty, a significant
impact on your result. Soconsideration of the whole face,
we want it to be harmonious, with your face, the anesthetic

(01:05):
result. So what can we do tomake your the ideal result being
the ideal candidate?
Well, we listen to what you haveto say. Do you want tip work
done? If you do, most peoplehave a certain position of the
tip that they want. Do they wanta certain rotation? Do they want
narrowing?
Do they want, a little bit ofsuper tip depression? Do they

(01:27):
want their bridge taken down? Dothey want it narrowed? So all
those considerations areperformed during your aesthetic
consultation. So everybody whowants a rhinoplasty that has a
concern on their aesthetic, appearance is an excellent
candidate.
In terms of what we do is basedon your feedback with me during
the consultation. So what wewant to get the best result

(01:50):
without a revision rate ingeneral, the American Academy of
Facial Plastic andReconstructive Surgery shows the
revision rate is about eight toten percent in The United
States. Our goal is to do theprocedure so you don't need a
second procedure to get theresults you're looking for. So
the most important thing isgetting good structural support
to your nose, that your nose isstable, that's not gonna droop

(02:13):
or push to one side or theother. So cartilage grafts are
important in that regard.
We either take it from the nasalseptum or the ear. And then for
the aesthetic appearance to yournose, get to get the ideal
result, sometimes we will morphyour nose. What does that mean?
We take a photograph of you froma side view or a bleak view. We
then change it with a softwareprogram so you yourself can see

(02:36):
what the end result will be.
So we use your input after thatmorphing that photograph so we
know how much to take the thebridge down. So morphing also
helps determine if you need achin implant. If you have
augmentation of your chin, oftenit makes someone who has a large
nose significantly smaller. Sophotomorphing can help you see

(02:58):
what your nose is gonna looklike so we can get the ideal
rhinoplasty that you're lookingfor. If you have any questions,
please contact our office, withthe underlying contact
information.
