A in the morning, well you know that it's either
fraudulent or the chemist doesn't know what they're doing, or
they don't have a plastic surgeon that reads chemistry, because
you know, these things get destroyed by ultraviolet light within
seconds of going out. So I call them the vampire ingredients.
You know, you put them on at night and they
stay on all night long, and really help your skin

look as good as possible. So that's what we're giving

They sent out eighteen warning letters to different companies for
putting botch of lineum toxins. That's like botox on their websites,
selling to you as a consumer without a prescription. We're
going to talk a little bit about that in a

losing weight. You know what did one of those government
officials say this week?

Yeah billion, I don't think billions, but lots and lots
of weight is being lost by a virtue of being
on ozempic. And zetbound and all those other GLP one drugs.
But the consequence of this rapid weight loss is what
we've been calling for the last few years ozempic face. Right,
that's where you get the sunken inface. And I've been

doing so much filler on women who have been on
zet bound and been on ozempic to fill in the wrinkles,
fill in those sunken in cheeks. Well, it also happens
to other parts of the body. You know, with rapid
weight loss, your skin doesn't get a chance to kind
of shrink along with the eight loss. Now, also when

you're young, when you're twenty or even thirty, your skin
is more resilient and has the ability to shrink relatively quickly.
Just think of pregnancies, right, you gain all this weight,
and you know a year after the pregnancy, your belly looks.

Well, not so lucky for most women, but a lot
of women do get shrinkage of the skin, but not

and this is news to Noah. Okay, so we are

the world. Okay, the numbers came out last week, but
what do you mean seven? Come on, let's get it

So ozempic it is so popular, zet bound so popular.
They result in major weight loss ten twenty thirty fifty
pounds in a very short time. And so many of
you are on these drugs. I know, you know you

(11:18):
can make judgments whatever, you know. Some people say, oh,
if you have ten pounds to lose, you shouldn't be
on it. We don't make those judgments. It's up to
you and your doctor, preferably an endochronologist like the folks
at well By MESSR. But let's talk about this. The
rapid weight loss that you get can cause real changes

(11:40):
in your appearance, and some are good and some are
not so good. So we call the ozempic face the
result of rapid weight loss. And I've been injecting filler
into the wrinkles and the cheeks in particular, and hiding
the jowls that are newly descended after you've had this
mess of weight loss. Well, we talked about that a

(12:02):
few weeks ago. I surprised Noah and talked about the
ozempic volvah okay on the show. We won't go into
that much today, but that's where we inject the filler
into the deflated lavia. Noah says, a hand on the
button there to censor. No, we're not going to censor.
This is a medical show. But also this week we're

(12:23):
going to talk about the ozempic breast. Well, let's talk
about for a minute what breasts are made of. We
all know, you know, they're esthetic units. They're very functional units,
of course, important for breastfeeding after pregnancy. They're made up
of breast tissue, which expands during pregnancy, and they're also
made up of fat, which often will expand during pregnancy

(12:46):
as you gain weight. But what happens is when the
breast tissue grows during pregnancy, it squeezes the fat, and
after pregnancy, when the breast tissue kind of goes away,
actually recedes and goes back to the pre pregnancy amount
that you have fat usually is less in your breast

(13:09):
than before pregnancy. Well, okay, that results in maybe sagging
breast because the skin has also been stretched out. So
what happens is, and this happens in pregnancy, and it
also happens as you get older, and it also happens
with rapid weight loss, the nipple begins to descend and
when you're a teenager, the nipple is in the lower

(13:31):
third of the breast and kind of points straight forward.
But when you've lost weight after pregnancy, your nipple begins
to descend and starts pointing down. All right, So that's
what happens. And you know this better than I. And
then you know you put on a bathing suit and
you say, okay, well, we're gonna need a backed bathing

(13:52):
suit because a bathing suit will not hold up my breast,
and I'll need a very structured bra, but a lot
of if you don't want to wear those. And so
the solution that plastic surgeons have is actually quite a
simple solution. It's called a breast lift. It's also called
a mastopexy mast TOPEXI. That's our technical word for it.

(14:13):
And it's an operation that I do many of and
we do it with you in a usually in a
surgeon center or in a hospital. We do it under
general antesesia. It takes about three hours to do this procedure. Now,
this is one where you have to be pretty good
at geometry, because it's applied geometry. What we're doing is
actually making a large, sagging, conical object into a smaller

(14:37):
less sagging conical object. And the way we do it
is by measuring between you know where the nipple should be.
I measure from the clavicle that's a collar bone, to
the nipple, and the nipple to the to the fold
underneath the breast, and it's all math. If you weren't
good at geometry, you probably wouldn't be good as a
plastic surgeon. So what I then do is I detach

(15:01):
the skin from the breast tissue. Now those of you
who are squeamish, you might want to not don't listen
for a couple of minutes here. But I detach the
breast from the breast skin, all right, and I leave
the nipple attached to the breast tissue at all time.
Some people take it off. I don't think that's a
good idea, because what we want to do is maintain
sensation and maintain the ability to breastfeed, and in most

(15:25):
cases both of those things happen. So if you wanted
a breastfeed after a breastleft, you should be able to.
If you and everybody wants to retain sensation in their nipple,
and we should be able to retain. And having said that,
there's always that small chance that you could lose sensation.
There's always that small chance that you could lose the
ability to breastfeed. Maybe you actually didn't even have the

(15:46):
ability to breastfeed. A lot of people can't produce enough
milk and have to supplement regardless. What I do then
is I remove the extra skin and match up the
volume of your breast tissue with the area of your skin,
and we use the skin to actually support the breast. Now,
if you're happy with the size of your breast, you

(16:06):
do not want an implant in. If you're happy with
the size of your breast, you don't want a breast reduction.
You just want a breast lift. We don't want to
touch the volume of your breast. We just want to
change the skin what we call the skin envelope. The
nice thing about this procedure is it actually doesn't hurt afterwards.
It really doesn't. On a scale of one to ten.

(16:28):
We're talking about a one to three maybe tile and
all of that new thing called gynaviics afterwards, no narcotics.
There are very few stitches that you'll see in the skin.
Probably fifteen years ago I changed my technique and I
use dissolving stitches all underneath, hundreds of them, and only
three stitches in the surface that you see that have

(16:49):
to be taken out about a week or so later.
And I use something called derma bond, which is a glue,
and that decreases pain with surgery, that decreases the infection rate,
and you're in decisions look significantly better than the days
when we put lots of stitches, or some people even
put staples in the breast. No, we don't want to
do that because then you have suture marks, you have

(17:10):
point tracks. Not a good idea. The derma bond is
really nice. You'll be able to go back to work
if you work just two days after this procedure. This
is not a disabling procedure, and you'll be able to
go back and you won't be going to the gym
for several weeks. You have to take it easy. But
the breastlift is a wonderful procedure. You will be able

(17:32):
to not wear broth you don't want to, and we're
an unbacked bathing suit if you want to do that,
and that is the wonder of the breastlift procedure. So
we're gonna take a short break. We're gonna come back
and we're going to talk about, Oh, what's going on

listening to the podcast. Also, all right, so botox. I
have been injecting botox since nineteen ninety seven. That's that's

(19:28):
a long time in the botox world. It was FDA
approved for cosmetic reasons shortly after it was considered off
label beforehand. Botox is botch alnum tox, and that's the
real name of it. It is kind of a wonder
drug in plastic surgery. There's a we're closing in on
ten million injections a year in the United States. It's

(19:51):
really quite amazing. I'm sure everyone has heard of botox,
but it's not just botox. Now there are six of
them there's botox, there's zemin, there's Disport, there's Javou, there's Daxifi,
and this new one, which is a very strange name
of Legbo. You know, not even pronounced or spelled leg Bo.

(20:12):
It's like latibo, but legbo whatever. It's a new one.
But there's six of them out there now, and they're
all very very similar drugs. They kind of break the
connection between nerves and muscles and then they work for
between two and four months for the most part to
get rid of wrinkles. And the most common areas that

(20:33):
I inject are the horizontal lines of the forehead, and
the vertical lines between the brows, those glabellar lines, the
eleven lines, and the crow's feet. And I also inject
an increasing number of chins, you know, the poe derons,
you know what that is, You know where the chin
looks like kind of an orange peel because the muscle
inserts into the skin and makes it kind of dimpled.

(20:56):
So it's really a nice thing to inject a little
bit of botox.

Speaker 3 (20:58):
There.

Speaker 5 (20:59):
I inject lines, what are the we have great names
for these things, those bunny lines on the sides of
the nose. I have started to inject the platisma muscle
the bands of the neck. I didn't want to do
it for a long the longest time, but now we
figured out exactly the right dose to inject in just
the right way to make it a safe procedure. So

(21:20):
I've been doing that, and now the newest one that
is backed by a lot of science is injection into
the massiter muscle. That's the muscle of the cheek that
you kind of clenched down on and if you clench
your teeth at night, you get this widened face, which
kind of looks okay in a man, but not so
good in a woman. And so women are now coming

(21:42):
in and asking for the mass muscle injection. And that's
one that lasts a little longer than the wrinkle reducing injections.
Wrinkles go away for maybe two three four months at
the most, but the master muscle that dec increases in
size and it takes about six months or so to recover.

Speaker 3 (22:03):
So those are the approved uses.

Speaker 5 (22:05):
Those are the ones that we do in plastic surgery
all the time, you know. And botox is very expensive,
you know, it is the chemical itself. I buy enough
for two people.

Speaker 3 (22:17):
Is in the.

Speaker 5 (22:18):
Six hundred and fifty dollars range. That's what I pay.
That's not even considering the cost of doing the procedure
and running your office. That's just the actual product cost.
So when something is that expensive, like wrinkle fillers are expensive,
it really invites fraudulent.

Speaker 3 (22:37):
You know products.

Speaker 5 (22:38):
So what's happened is that there's fake botox out there,
and we talked about that a few months ago, but
there's also a whole bunch of foreign companies and even
some American companies that are putting botox or botox like
substances botch a line umlike substances on the Internet that you,
as a consumer can buy. This bog my mind. I

(23:00):
have to tell you, the thought of you buying batchelinum
talksin getting a syringe and a little needle and injecting
it into yourself or your friends with no knowledge of medicine,
is just absolutely nuts.

Speaker 3 (23:14):
But it's happening.

Speaker 5 (23:15):
It's happening frequently apparently, And in fact, the FDA this
week on November fifth, sent eighteen warning letters to different
companies that advertise and sell to United States citizens right
here through the Internet. They sent eighteen warning letters and
they told those companies, you have better stop because we're

(23:37):
going to seize your product now when it comes into
the United States. They're sending it from other countries. They're
sending it mail order in these nondescript packages. But the
FDA is going after these. They've warned them you better stop.
So I checked tonight just before the broadcast. One of
those companies.

Speaker 3 (23:55):
I checked.

Speaker 5 (23:56):
You know they did they stop? Nope, Nope, you can still.
I'm not going to tell you which one, and it
is because I don't want to support the use of
these things. But you can buy bot It's not botox.
It's not even one of those six names that I
just mentioned the FDA approved products in the United States.
There's all these companies all over in Korea and China

(24:18):
and Panama and yes, even in the United States that
are making illegal product and selling it. I guess it
might be legal in their country, but it's not legal
in this country. They're selling it to you. Now, what's
the problem with this? Of course, you can die from
injecting too much of this material. You know, bachelinum toxin

(24:38):
is actually the most potent toxin known to mankind. We
use teeny teeny tiny it's a medical term amounts when
I inject it into you, and I do it in
such a controlled way, I know what I'm doing. I've
injected probably in the area of I tried to kind
of put this number together, probably in the area of
twelve thousand patients since nineteen ninety seven. I've injected both

(25:00):
talks into and the worst thing I've had is a bruise.
But if you injected, if you buy, you know, let's
get a couple of miles and inject it into myself
or your friends, terrible, terrible things, including death, canneccur. So
so don't do that. Listen to the FDA, all right.
