answers that you want, the answers to the questions that

are about cosmetic surgery, America's favorite topic. It really is
one of the favorite topics. You can hardly listen to
a podcast radio. Look at a newspaper. Do they have
those anymore? Without hearing about cosmetic surgery. You know, if
you saw the Grammys last week, well I'll bet you

saw a lot of cosmetic surgery. Well, I'm a board
certified plastic surgeon and that means I know a lot
about this subject and I'm here to to give you
information that will help keep you safe. So tonight's show,
we are going to be talking about eyelid lifts, one
of my favorite procedures, and then we're also going to

that's a blefhoplasty for those of you who want the
technical terms, It's an eyelid lift to you, blefheroplasty to me.
We're going to talk all about making your eyes look better.
And you know, that's something that I've done as early
as eighteen years old and as late as probably eighty five.
I think is the oldest blefferoplasty I've done. So we're

going to talk about that. We're going to talk about
something that Noah will really enjoy tonight. It is the
stupid one of the stupidest things. Oh, you find these
things all the time in plastic surgery. This is really ridiculous.
So I'm going to talk about taping your face. Taping
your face to get rid of wrinkles. Oh, okay, you'll

want to stay tuned for that one. And we're also
going to talk about another topic of mine, the things
that I don't do in plastic surgery. There are a
lot of procedures that I do do and some that
I don't, and I'm gonna explain to you why I
don't do kuy bella. Oh what is ky Bella? Well,

let's start out with Kuybella. We'll kind of warm up
with that one. Kuybella.

What is that? Yeah?

Speaker 3 (04:23):
You know it is something that sounds really good, right,
inject a chemical into fat and watch it disappear. Right, Well,
you know what, it was called mesotherapy fifteen or so
years ago. It was an unproven procedure, but then there
was enough science to deoxycolic acid. That's what Kuybella is.

(04:45):
It's actually a bile acid. Bile dissolves fat, right, remember
that from biology. Well, if you inject bile or deoxycolic
acid or Kuybella into fat, it does make it go away.

Speaker 4 (04:59):
So sounds good.

Speaker 3 (04:59):
So so right, who doesn't want a non invasive way
to get rid of fat? Because otherwise we have to
do liposuction, which is a surgical procedure, or some other
way directly open up the skin and remove fat. That's
the way we did it before liposuction. Coybella sounds good, right, Well,
you know I was I trained in it like everybody else,

(05:20):
and then I kind of watched. I watched a new
procedure and it does work, although it takes a bunch
of different procedures, like three or four episodes at a
few thousand dollars each one. If you add up the
amount of money you're spending on Kuybella, you probably are
spending more than you would if you were to have

(05:40):
liposuction fat removal surgical procedure from your chin, neck and
jal area. So okay, first of all, is it cost effective? Well,
you know you can decide on that because a lot
of people simply don't want to have surgery.

So that's okay.

Speaker 3 (05:57):
But but what are the problems that have been seen
with Kuybella. Well, here's one in our journal this month.
Now this is something that was FDA approved in twenty fifteen,
but there were nerve injuries found in a nerve rather
in a journal article, four percent of people wound up

(06:19):
getting injuries in their nerves. And I'll tell you that's
a lot. If you get an injury to the marginal
mandibular nerve. That's the one that controls your mouth. You're
gonna have problems smiling and other problems. And I don't know,
be careful because for only two hundred ninety nine dollars people,
that means doctors, nurses, maybe veterinarians can train in Guypella

and get a certificate.

you know I'm a Board certified plastic surgeon. That means
I'm I'm real. I do this during the week. I
sit there in my office on Park Avenue and in
Somerset County, New Jersey. I inject botox, I inject fillers,

I do facelifts, eyelid lifts, rhino plastics, liposuction, tummy tucks,
breast surgery, all that sort of thing. That's what I do.
And on Saturday evenings. I'm here in the studio at
WOR for nearly twenty years, trying to educate, trying to entertain,
and I tell you I love radio myself. I've been
doing radio since I was president of the radio club

in high school in Highland Falls, New York, a long
long time ago. That was an age ago, and then
at Rutgers Radio and now here on WOR for.

A long time also.

Speaker 3 (09:06):
So but during the week i'm you can find me
in my office and you can actually make an appointment
to see me at my Park Avenue office. It's two one,
two seven five three, eighteen twenty or in New Jersey.
But what I like to do is operate, and you'll
often find me in the operating room at either Manhattan
nine Year or Robert Woo Johnson University Hospital. And I

(09:27):
like doing islid lifts. That's one of my favorite procedures. Interestingly,
I did one on CNN Radio when I was a
resident way way back in the nineteen eighties. I remember CNN,
not radio TV rather, and CNN was a new station
at the time, and they were doing a program on

(09:48):
cosmetic surgery by residents at the University of Chicago, where
I trained, and my chief said, well, you're the chief resident,
so let's have you do one. And so a little
intimidating when you're a new ee and you're trying. You've
got millions of people watching you while you're operating, and yeah,
I've done so many, thousands of eyelid lifts. Then I'm

(10:10):
happy if CNN wants to come and watch me and
compare the tapes, I think you'll see an interesting progression.
So I love doing eyelid lists. So here's why we
do them. As you get older, you tend to show
aging first in your eyelids.

Speaker 4 (10:25):
It really is.

Speaker 3 (10:26):
You know, you look in the mirror and I have
had people in their thirties tell me, well, you know what,
I've got wrinkles now and extra skin.

Speaker 4 (10:34):
I didn't have it a.

Speaker 3 (10:35):
Few years ago, And I'll say, well, you know, often
it's not something that occurs with aging, it's something that
occurs genetically. And that means I've seen teenagers have full
upper lids and not a defined crease of their upper lids.
So I've done Blepherplass that's the name of the operation

(10:55):
on people, usually women as young as eighteen years old,
but for the most part It's an operation that begins
in your forties, and I've done them in forties, fifty
year old, sixty year old, seventies, and even eighty year
old people. Here's what happens as you get older, and
you just have to look in the mirror to see
this happen. You get extra skin of your upper lids.

(11:17):
Your brows tend to droop just a little bit as
you get older, So they start out when you're a teenager.
You know, in a woman, your eyebrows are at or
above the bony rim, and in a man they're at
the bony rim. So if you're not driving, feel your eye,
feel your eyebrow, feel your bone, and you'll see that
your your eyebrow hopefully is right around your bony rim. Now,

(11:40):
the problem is, as you get extra eyelid skin, you
tend to lift up your eyebrow. Your brain wants to
get as much light in your eye as possible, so
you lift up your eyebrow and you wind up getting
those horizontal.

Speaker 4 (11:53):
Creases of your forehead.

Speaker 3 (11:54):
And if I botox those, it'll drop down your eyebrows,
drop down your forehead, and you'll look and you'll say,
wait a minute, you botoxed me, and now I have
extra skin in your eyelids. Well, you had it all along,
and the botox just exposed it to you, because now
you cannot lift your brows as well. So a lot
of people have to make that decision do you want

(12:15):
botox to get rid of those wrinkles or do you
want to maybe have an eyelid lift. So when I
examine you, I'll have you actually close your eyes and
try and relax your eyebrow, and I want to see
the real position of the eyebrow on your face. If
your eyebrow is below the bony rim, then we might
consider doing a brow lift as well as an eyelid lift.

(12:39):
So then you have you open your eyes, I show
you the photographs and we go through the process of
a consultation for eyelid surgery. I look at the extra
skin of your upper lids, and then I also look
at how much fat you have. And most people get
this puff of fat right by their nose in the
corner of their eye, and it's very important that a
plastic surgery moved that when we do an upper eyelid lift,

(13:01):
because if I don't remove that fat, or I miss
the fat and I remove the skin, it'll be even
more obvious afterwards. So a lot of people think, well, okay,
we have two eyelids. Nope, nope, you actually have four eyelids.
There's two uppers and two lowers. And it's such a
common thing. I have so many patients that say, what
do you mean I have to do upper and lower eyelids.

(13:22):
I thought there's only uppers. Your lower eyelids are a
little different than your upper eyelids. They tend to droop
as you get older and show more of the white
of your eye. And so when I examine your lower eyelids,
I lift the skin off of your eyeball and I
see how fast it takes to snap back. And if

(13:43):
you have a slow snap back, then we might have
to do something a little extra. When I remove some
skin from upper from your lower eyelids rather so a
lower BLEPHROPLASTI that's what it's called. Involves making an incision
right underneath the eyelashes, and then I lift up the
skin and I expose those three fat pads from your
lower lids, and I remove just the right amount of fat.

(14:05):
If I removed too much, well you'll look like you
have a sunken in eyelid, and if I leave some
you'll say, hey, you didn't take enough. And there's a
rule of thumb when I do eyelid surgery, and that is,
however much skin I think I need to remove, I
remove just a little bit less.

Speaker 4 (14:21):
And the reason is there's no leeway here.

Speaker 3 (14:24):
If I take too much skin, then you might have
that basset hound appearance afterwards.

Speaker 4 (14:29):
And what does that mean.

Speaker 3 (14:30):
It's a condition called, all right, write this one down.
An ectropion. That's something you don't want. An ectropion is
a pull down of your lower eyelid. And if you
get that, well, you're gonna come back to me and
say something's not quite right. And hopefully I don't do
that because if you're at a a little bit of
higher risk for getting a pull down, and that is

(14:51):
if you have a lot of laxity of your lower lid,
if you get older, the older you get, the more
likely you are to have this. And at about half
a patience, I do something called a canthopexy that's a
belt tightening procedure. So if you look at your lower
lid as if they were a pair of pants and
you had creases in your pants and you wanted to
straighten them out. You pull on your pants to straighten

(15:14):
out the creases. If your belt is loose, your pants
fall down. So what do you do. You tighten the
belt and then you can pull forever on your pants
and they won't fall down. Same thing with your eyelid.
I need to tighten the belt of the lower eyelid,
and that is called a canthopexy. That's the name of
the operation. And I do that, as I said, in
about half of people. So that's what we have for

(15:36):
an eyelid lift. I usually will do the upper lids
and the lower lids and the result is, well, you
look better and actually you can see better in many
cases because when you have all that extra skin of
your upper eyelids, when it kind of it takes away
from the light that gets in your eye, and when
let's say you're driving and you're driving up a hill,

(15:58):
that skin of your upper lids can actually interfere with
vision and almost like having a baseball hat on while
you're driving. So then you might say, well, would insurance
pay for this because my vision's interrupted? And sometimes yes,
usually not it has to really be affected, but sometimes
insurance will pay for an upper eyelid lift skin only,

(16:20):
not for removal of the fat, and certainly not for
the lower lids. So that is, in a nutshell, an
eyelid lift one of the more common procedures that I do.
And yeah, it's local anesthesia. It's about takes about maybe
two hours or so to do the procedure and you're
back to work five days maybe six days later, as
long as you're willing to wear dark glasses. All right,

So I talk about cosmetic surgery on this show. I
talk about the procedures that I like to do. I
talk about the procedures that are kinda a little wacky,
and I also talk about things that are not so wacky,
like kaibella. That's not a wacky procedure, but I don't
do it because of potential drawbacks by the procedure. But
I saw one this week that really is kind of funny.

(18:41):
And I know Noah is my engineer for almost twenty
years now, and he loves these things.

Speaker 4 (18:47):
So this one's for Noah.

Speaker 3 (18:48):
So have you ever heard of people taping their face? No,
taping their face to get rid of wrinkle, taking actual
tape to get rid of wrinkles? I mean, how does
that work? It's all over the internet this week. You
just have to look and kind of crazy. And there
are people that show these pictures and they put this

(19:08):
big V shape piece of tape between the brows up
to the forehead, up to the hairline, and then these
other two pieces of tape on the nasal abial folds
and the inner cheegue. And it's supposed you're supposed to
go to sleep with this tape and it's supposed to
train your face train. Sorry, sorry, I have to laugh.

(19:30):
It's supposed to train your skin to have the proper
nutrients and oxygen to reduce puffiness and lines. That's what
they say. All right, how could this possibly work? You know,
I look at these things. When I do a new
procedure or when I do an old procedure. I want
there to be science, I mean real science. I want

(19:51):
there to be scientific publications, clinical publications. I want to
do for you what really works, and I don't want
to do procedures that are trying to be nice here,
not so valuable and certainly not scientifically proven. So so now,
how can taping your face possibly work? Where would someone

(20:13):
get the idea? And they say, well, you know, it
follows the lymphatic system and the muscle structure, and there's
all this gobbledegook.

Speaker 4 (20:21):
So let's go to the science. There is none.

Speaker 3 (20:25):
There is none, there's none. There's not a single scientific paper.
Well there was, all right, let's back up a bit
in two thousand and three. In two thousand and three,
there actually was a scientific paper, but it was different.
It was out of the University of Chicago, where I trained,
and it was using surgical adhesive tape for facial rejuvenation.

(20:49):
But the way they did it. They took this Scotch
magic tape, the three M stuff, you know, you use
it food for I don't know, Christmas packages and things
like that. So this eighty seven year old woman who
had a prior cardio pulmonary arrest that means she nearly died,
she had a brain aneurysm. She went to the cosmetic
surgery clinic at the University of Chicago and they said, well,

(21:12):
you can't have surgery, and then they said, well, how
about just taping your site. You want to tape your
skin up so that it looks better. And she did
it and they.

Speaker 4 (21:23):
Took pictures of it.

Speaker 3 (21:24):
She used this adhesive tape every single day, and she
often even forgot that the tape was on her face
and she put her makeup over the tape and that's
what she did. But that was to lift her cheeks,
lift her brows, lift her neck and kind of crazy,
but it worked for this eighty seven year old woman
who could not have any procedure at all. But that

(21:46):
is probably where they got this idea of actually using
tape doesn't work though, I mean, if you just put
it on at night and take it off during the day,
the tape isn't lifting anything. It's just another one of
these idiotic At least this one doesn't cost a lot
of money. So you can take that tape and just
use it on your paper. You don't have to use

(22:06):
it on your face anymore. All right on board, certified
