you're having good weather. Also, you know the summer's not over.
It's a UV index of six today in the New
York area seven tomorrow. And what does that mean? That
means you still need your sunscreen, and boy, I'll tell you,
as a plastic surgeon, that is very important to me
and my patients because it is the ultraviolet light that

(01:53):
causes premature aging of your skin, and it's one of
the few things that you can do to actually slow
the aging process as long as you don't smoke. And
I can't imagine that my listeners smoke if you want
to look your best, and but my listeners do you know,
do go out in the sun a little bit excessively.
I've had so many patients that do. It's hard to

(02:15):
give that up. But it will prematurely age your skin.
All right. Today we are going to be talking about
procedures that men have, you know, and kind of as
a couple. You know, I've got a lot of men
that come in with their wives and it's very interesting
the dynamic between their procedures and their wives procedures. So

(02:35):
we're going to talk about that today. We're also gonna
we're gonna mention some of the problems with the cosmetic
surgery chains which seem to be growing around the United States.
And we'll talk a little bit about al thera, which
is a very nice procedure that's completely non invasive. There's
some new evidence that all thera does quite a bit

(02:56):
and maybe a little bit more than I anticipate. All right,

really is. It's all you need at night before you

three or four or even more different products to put

at night. I know you have them. Maybe it's about
your jowls. Maybe it's about wrinkles, that's what we talk
about on the show. Maybe it's about droopy breasts or
fat on your hips, or you know, a per tuber
and belly after pregnancy. That's what we talk about. I'm
a board certified plastic surgeon, host of this show for

twenty years with Noah in the studio. Today I'm broadcasting
live from the Hamptons, which have quieted down quite a
bit in the summer. It is so busy in the
Hamptons today. It's a kind of a ghost town out here,
all right, Noah, Let's get right to it. Couples procedures,
you know. I had a man last week who came

(05:25):
in and he's interested in having a what I call
a male facelift. It's different from a woman's facelift because
men don't do well with incisions around the ear. So
when I do a facelift and a man, it's really
a necklift and with some modification. So a traditional facelift

(05:45):
is it's a poorly worded procedure. It is a cheek
lift and a necklift. And so I do what's called
a short scar facelift, which has shorter incisions. We don't
make incisions in the hairline, which is good for both
men and women, because if you have an incision inside
your hair, you will most likely see it if you

(06:06):
keep your hair on the shorter side, or if you
go swimming, or if you come out of the shower,
or if you're a woman you want to wear your
hair back or in a ponytail. These new facelifts, these
short scar facelifts are called ponytail friendly facelifts because there's
no incisions in the hair. If you take a good
look at former President Biden, and I analyzed his photographs

(06:29):
for a newspaper, I believe according to the photographs anyway,
he had a facelift, and there's a big scar in
his hairlined behind his ear. Well, the new facelifts don't
leave that scar. There's just a small scar in front
of the ear, looping back just a short distance. But
in men that's a problem too. And women, well, you know,

(06:51):
women are used to covering scars with makeup. They have
longer hair, easy to cover. But in a man, well,
that scar is a problem because one, men don't like
wearing makeup, and two, well, it brings hair bearing skin
into the ear, and that's a problem. If you're a man.
You know, it's hard enough if you've got hair growing

(07:12):
in your ear, no less bringing the true beard into
your ears. That's not so much fun. Right, So men
don't do real well with those, and that's one of
the reasons why facelifts are not particularly popular in men.
But what is more popular is the necklift, which is
there's just a small incision underneath the chin, and through

(07:32):
that in decision I suction out the fat, I bring
the muscles together to give you a better contour. I've
got one coming up where I'm also adding a chin
implant because men like bigger chins. And then, of course,
you know, we want to remove as much fat as
we can, but not too much, because then you look
like a muppet, right. You know, you just have to

(07:54):
walk around the streets of Manhattan to see what I'm
talking about over done facelifts. There's artwork to doing a
facelip properly in both a man and a woman. We
know that, but it's a different operation because we want
to leave a little bit more fat in a man
than a woman so as not to feminize the neck.

(08:15):

I do husband and wife couples really very interesting. And
the man comes in and then the woman decides, well,
wait a minute, now, my husband's not gonna look that
good without me looking better, so maybe we'll do this together.

All right, So today we are talking about his and
her cosmetic surgery. Really, you know couple cosmetic surgery. Across

(11:02):
the United States, plastic surgeons are they're trying so hard
to get men to have cosmetic surgery, you know, because
ninety to ninety five percent of cosmetic surgery is done
by women, and that leaves an enormous population of people,
you know, so of course good marketers and cosmetic surgery
want that other half of the population. But you know,

(11:25):
most men just don't want a lot of the procedures
that women want. But I do get men, and most
of the men who come to my practice are the
spouses of people of women who have already had let's
say a facelift or something like that. But recently, in
the last year or so, I've had more men come
and bring their wives in. It's always very interesting, and

(11:48):
I've had men have filler and then their wives have filler,
and I've had men have their eyelids done and then
the women have their eyelids done. And a couple of
weeks ago, I had a man that came in for
BOTA and he's scheduled to have his neck operated on,
and his wife is scheduled now to have botox and
to talk about other procedures. It's pretty common because the

(12:10):
benefits of cosmetic surgery are seen up front and personal
and it seemed that, you know, it's not so mysterious
if your spouse is having it, and also you know,
not that bad, right. You know, no one wants surgery, right,
But when we're having cosmetic surgery, particularly surgery under sedation

(12:31):
as opposed to general anesesia, and recovery is usually quite
rapid after cosmetic surgery. Well, you know, I don't want
to be little it. It's a real procedure, you know,
when we're talking about facelifts and eyelid surgery and liposuction
and things like that. But it's certainly not heart surgery,
it's not joint replacement surgery and things like that, and

(12:52):
the risks are less and the benefits are quite clear.
You look better very rapidly. So the procedures that men
have In two thousand and four, the American Society of
Plastic Surgeons, they compile their statistics and they divide their
statistics into the non invasive procedures like filler and botox,
and then of course the more invasive procedures like facelifts

(13:16):
and tummy tucks and things like that. So let's take
a look at what men have had in the year
two and twenty four that would be last year there
were around six hundred thousand men that had botox. That's
pretty good, But of course there were nine point eight
nine point nine million botox procedures, so certainly women dwarf men,

(13:39):
but still it's a sizeable number six hundred thousand men.
When we talk about lasers, now, this is sort of
surprising because women had three something million lasers last year
and men did have half a million, which is sort
of surprising because there is downtime for laser procedures. But
I think a lot of that is a laser tattoo

(14:01):
removal and laser treatment of capillaries, both in the legs
and the face, because a lot of men do get
roseaesia and roseesia. You know, that's a condition of the
skin where you get this what we call a butterfly
rash of the face, you know, of the cheeks and
the nose. Of course, that across that distribution, and it's

(14:24):
brought on by spicy foods and alcohol and smoking and
things like that. It's a condition that's most commonly treated
by a dermatologist. But I get involved with the endpoint
when you start getting red veins of your face or
something called rhina fime. I saw someone last week with

(14:45):
rhina fhyma of the nose. That's what it is. It's
a thickening of the skin of the nose after years
and years of roseaesia. And so what we want to
do is try and prevent that from happening. But that's
not always possible.

Speaker 5 (14:59):
You know.

Speaker 3 (15:00):
The dermatolosis work pretty hard with antibiotics and all sorts
of skin creams to control roseatia. I see you when
the red becomes quite visible, and I'll beat down those
capillarias every year or two with the YAG laser. But
if you do develop thickened skin of the nose with
that rhina feima, then what we do is either laser

(15:22):
the skin or we shave the skin down so that
we contour the skin a little bit better. We've got
flow on the line. Flow. What can I do for you?
What is your wrinkle?

Speaker 5 (15:33):
Yes, hello, doctor Perry. I have a question, which is
after a procedure a facelift I neck where there was
that transfusion place in different places including the jeeline. If
I feel that it's too heavy looking, is it possible

(15:54):
to remove that fat?

Speaker 3 (15:56):
Yes? So how long ago did you have the facelift.

Speaker 5 (15:59):
Flow about two years.

Speaker 3 (16:02):
Okay, and how old are you?

Speaker 5 (16:05):
Oh, sixty four?

Speaker 3 (16:07):
Yeah, to think about that, Okay, all right, so you're
sixty four years old, you had a facelift. You know,
there's so much artistry to facelifting. And one of the
things that is in vogue now is fat grafting. And
you know, it's not new, by the way. I started
doing fat grafting in my residency in the nineteen eighties,
believe it or not, but it's gotten a little bit

(16:28):
more refined. We do this nano fat now, so it's
not chunks of fat. How's that. It's a kind of
a more liquid fat that we inject now. But because
it's readily available, and you know, a lot of plastic
surgeons have incorporated into facelifts, and sometimes it's indicated and
sometimes it's not, and there's so much artistry to it.

(16:50):
If too much is put in, then you get kind
of a chubby appearance to your face. And that's what
sounds sounds like happened to you. So the answer is,
it is usually possible to remove that fat. Usually it
can be done under just local anesthesia with what I
call micro liposuction. So that means we numb it up,

(17:12):
and it's usually done in the office when we're just
trying to remove a little bit of a fat that
was injected, and we numb it up and we use
these little tiny canulas, little tiny tubes to remove just
enough because when we do fat grafting, we're not putting
a lot of fat in there. We're putting a little fat,
but it doesn't take a lot to change the angles

(17:34):
of the face right and the width of the face
so that it looks kind of funny. Is that your case? Yes, yeah, yeah,
So that can be done not necessarily requiring another facelift.
Sometimes it is required because sometimes it is not possible
to just go ahead and suction that out if there's

(17:55):
too much scar tissue, or depending on the location of
where that was put, some times we actually have to
make incisions, lift up the skin see the fat and
remove it directly. But that is determined into consultation, and
usually it's a it's uh, you're we're able to remove
the extra fat, and sometimes we can do it by

(18:16):
injecting just a little bit of steroid or something like that,
or even doing something called altherra to destroy the fat
without even making an incision. So there's a few things
that we have up our sleeves. Sometimes we suction it out,
Sometimes we try and dissolve the fat. Sometimes we might
destroy the fat with different techniques. So a consultation, during

(18:38):
your consultation, we'd make that determination and trying to give
you a hand to look as good as you can.

Speaker 5 (18:45):
Okay, flow, is the body fat dissolvable by ultra and others?

Speaker 3 (18:52):
It is? Probably is? It probably is. So you know
the interesting thing is where do they take the fat,
by the way, from Was it from your best Yeah?
So that fat will will follow the genetic destiny of
where it came from, not where it has been put.
So that means if it's thigh fat and you have

(19:15):
maybe decided to gain a little bit of weight, a
few pounds, and you normally would put it on your thighs,
now you're going to put it in your face more
so than you would have otherwise. So that fat thinks
it's in your thigh. So when you have that pew, Yeah,
that's the thing, and that may have been what happened
to you. If you have kind of a little bit

(19:38):
of extra fat on your thighs and you know that
when you gain a few pounds, you gain it in
your thighs. Now you're going to gain it in your face.
So we do take that into consideration. Is that do
you think that's what happened to you?

Speaker 1 (19:52):
No?

Speaker 5 (19:52):
No, in fact, I think I lost a little weight.
I just think maybe I've read that the fact transfusion
your own bodies that takes better in the jaw area.
Some might dissolve some stays, but it seems perhaps more
state or all of its state. I don't know.

Speaker 3 (20:10):
Yeah, it's extremely variable, it really is. So I've had
patients that I have documented that the fat has survived
two decades later. And I have seen patients where I've
injected fat and it's gone in six months. So it's very,
very variable, much more variable than let's say, wrinkle filler,

(20:30):
which is so predictable you know that it will always
go away, but it will always be there, you know,
nine to fifteen months, you know. But with with filler,
with fat filler, and it's transplanted, I think you use
some other terminology. It's transplanted fat, fat transfer or transplanted
fat or fat grafting. Those are the proper terms for it.

(20:54):
But regardless, it if it's there at about a year,
it will be there forever. The most part, all right, flow.

in the podcast world. Okay, we were talking about couples,
plastic surgery and the things that men like to do.
You know what's kind of interesting is that wrinkle fillers

(22:51):
so popular five and a half million women last year
at wrinkle fillers not nearly as popular in men, about
two hundred maybe two hundred and forty thousand. So that's
a huge, huge difference in and one of the reasons
I think is when I put wrinkle filler in men,
I will rarely put it into wrinkles. Actually where we

(23:14):
put it is the nasalabial folds and the marionette lines
and the jawline. But men don't have those fine wrinkles
that women have most of the time, but they have
grooves in that sagging and we can use wrinkle filler
for that. So the the surgical procedures that men have,
those are the ones where you know, we actually make

(23:36):
an incision, we go to the operating room, you have
some sedation or general anesthesia. So for men, well, what's
the most popular one. It is actually liposuction. About twenty
one thousand men last year had liposuction. Now that's not
a huge huge number the men I've done. The two
areas are the belly, you know, the abdomen and flanks.

(23:59):
That's probably the most common area men, and of course
the neck, neck and jows very popular in men and women.
Those are the two areas. Very rarely do we do
suction anywhere else in men. Now that's not including male
breast reduction, where they are about twenty six thousand, but
that's kind of a different category because that's open surgery.

(24:20):
We have to actually do what's called a mastectomy, removal
a breast tissue and suction the perimeter there. So liposuction
that is the most common procedure, followed by eyelid surgery
that's called the blephoplasty. Fifteen thousand men in the United
States compared to about one hundred and twenty thousand women.
But when I do a man, I'll often do his wife.

(24:43):
It's very interesting because they'll come in one will decide
to do it though like the result. They'll see that
having eyelid surgery is a pretty simple thing to go through.
It's not particularly painful, the recovery is pretty fast, and
then the spouse will have the procedure. It works both ways, right,
So blephroplasty that's the second most common. And then really

(25:05):
there are not other procedures that have great, great numbers.
You know, about seven thousand rhino plastis and men not
a lot compared to women there's about fifty thousand or so,
you know. When we get into other procedures, very very few.
So it's it's really you know, lip a suction. As

(25:27):
we discuss the surgical procedures, botox in the non surgical procedures,
all right, well, we're not going to get to our
