So I hope you've all had a great week. We're
really entering the spring season now. The spring season is
upon us, and the weather is getting warm. Today it
was a UV index of seven, and do you know
what that means? What's a UV index of seven? A
seven means if you go out in the sun for

(02:38):
twenty minutes, you are going to fry, especially this time
of year. You know, here we are. If you're listening
to the podcast in September, oh, who knows what's going on.
But if you're listening now live uh or maybe in
the middle of May, well, then you know the the
UV index means a lot to you. Your skin is

(02:59):
at its lightest for the year because you have gone
through a winter, a tough winter, right, and that means
your skin has been kind of gotten light. The melanosites
in your skin are not pumping out melanin or brown pigment,
but they will in a few weeks. So your skin
is very light. You need sunscreen. You need an SPF

(03:22):
at least fifteen sunscreen every day that the UV index
is over three, and today in New York it was
seven between eleven thirty. I check these numbers between eleven
thirty and two forty five today and before that it's
a little less. But you really need sunscreen pretty much
all the sunny hours of the day from now, write
this on your calendar until until Halloween. You can pretty

(03:45):
much forget about the sunscreen after that. Also, although some
dermatologists would yeah, they would argue, they'd say year round,
I don't think. So. You need a UV index over
three and we're gonna be giving out Yes, it's a
good segue, right, we're gonna be giving out bottles of
my daytime. Daytime is an SPF twenty. It's actually twenty three,
but we mark it down. You never I can round up.

(04:07):
The FDA says you can round down, but not up.
It's an SPF twenty three sunscreen. It's got zinc oxide.
You know, a couple of women came to the office
yesterday from San Francisco and had consultations, and I gave
them a long, long lecture on sunscreen and skincare because
so many people do it wrong. So we'll maybe mention

(04:30):
a little bit more about sunscreen. But we are gonna
talk I promised you we'll talk about exizomes. We're gonna
get to those today. Well, if we get time, we'll
talk about red light therapy. What on earth is this stuff?
We're gonna talk about the biggest bang for your buck
procedures in plastic surgery. About once a year I do this.
It's a lot of fun to talk about those. Those
are the procedures they give you the biggest result for

(04:53):
the least cost. So we'll talk about that. It's only
a half hour show. Maybe baby rhinoplasty, what is that? Oh,
there's so many things to talk about, but let's go
to our first phone call. Sondra. Sondra, what can I
do for you? What's your wrinkle?

Speaker 3 (05:08):
Hello? Dr Perry, Doctor Perry. I meet people every day.
You know, pretty outgoing person, and I meet women that
would love to maybe do a nip and a tuck,
but they're afraid. They're so afraid it's such a major,
major step, and then what happens if they don't like it?
Have you ever encountered that kind of scenario?

Speaker 5 (05:30):
Well, you know, I mean, look, we can't guarantee results
in plastic surgery. It's not like selling a car, Sondra,
we can't. And I understand people's trepidation. You know, there
are some risks involved. And again, it's not like you know,
you gets a dent in your car, you bring it
to your dealer, two weeks later, maybe less, maybe more,

(05:53):
you get your car back and it's perfect. Not so
with surgery. So it's so important to choose the right surgeon.
I've been doing this. I finished my residency in nineteen
eighty seven. I've been doing this for thirty something years.
I love operating. I'm a board certified plastic surgeon. I
trained at good places. So you know, when you choose
a plastic surgeon, you have to stack your cards, you know,

(06:14):
in your favor. If you go to someone that's not
board certified, you don't know what you're getting If you
go to someone that might not have trained at a
good place, then you don't know what you're getting if
you decide you're going to I'll tell you something. I
am operating on someone this week who had surgery by
someone else, not cosmetic surgery in Mexico. You know, she's

(06:36):
having her surgery with me this week, cosmetic surgery, but
she went. And you know, I always wonder, you know,
you can't speak the language, you can't communicate, and how
do you know what's going on in another country? Mexico,
South America, Turkey, India, wherever you're choosing to have your
medical tourism to save a few dollars. You know, I

(06:57):
understand the value of saving money. You save it on
a washing machine, you save it on clothing. But when
you go to another country, can't speak the language, have
no idea what the credentials of the surgeon are in
that country. And you know what, I'll tell you a
dirty little secret, Sondra about once every couple of years,
I get an email from a company and they say,

(07:18):
you know, do you have any expired products like suture
and other other things, drugs and things like that that expire?
And of course we throw those out in our office.
But those companies buy for five to ten cents on
a dollar expire products and they say, well, you know,
we're using those for research or maybe veterinary medicine. You

(07:41):
know what's happening. They send them to South America, they
send them to Middle you know, the Mexico and other countries,
and they're used there. They're used there. You know, probably
they're okay because a product that expired two weeks ago
doesn't make much of a difference. But you know, the
FDA puts expiration dates on these products for good reason,
and you know we must adhere to those dates in

(08:04):
this country. So anyway, Sondra, back to your question, how
do you know you know you're going to get a
good result, Well, no one can guarantee that. In any
plastic surgeon that tells you I guarantee you a perfect result.
You know, my chief at the University of Chicago used
to say, there are two types of plastic surgeons that
do that, and first are liars and the second are

(08:26):
our people that have not done a lot of surgery
because they you know, they've only done three rhinoplastys, so
you know, all of them came out perfectly, but the
fourth one, who knows what it could come out like.
So anyway, Sondra, you have to stack the odds in
your favor. When you go to a plastic surgeon, hopefully
it's a trusted plastic surgeon, a board certified plastic surgeon,

(08:46):
someone that's trained at good places, has hospital privileges at
a good institution. And I have privileges at Lennox Hill
and Manhattan iron Ere and Robert wood Johnson in places
like that.

The uh.

But you want to do that, you want to stack
in your favor. You want to check the database of
the state. Every state is a database, so you can
learn whether or not your doctor has been sanctioned, has
had a reprimand or a suspension, or even a revocation. Yeah,
they could have had their license revoked and then they
could have gotten it back. I spent ten years on
the other side of that, on the Board of Medical

(09:19):
Examiners in the state of New Jersey. We were the
ones that disciplined doctors, so I know all about that
and how doctors that might not be the best doctors
sometimes get their licenses back because they have very good lawyers.
But anyway, that's the story, Sondra, you want to stack
it in your favor. See the photographs that the surgeon
has to show you. Yes, you can talk to patients.

(09:41):
Most surgeons will have other patients that you can talk
to who have had the procedure that you want, and
then you know. But from there, we can't give you
one hundred percent guarantee, but you certainly have odds stacked
in your favor. Okay, it sounds good.

I was at a function this week, met a lot
of my listeners. That was a lot of fun. Thank
you so much for listening, mostly some women at a
political function we were listening. Actually heard Alan Dershowitz. He
was a very interesting speaker this week. So you hear
him on the radio all the time. It's a lot
of fun, all right. So have you heard of the

(12:23):
baby rhinoplasty? Have you heard of that? Noah, the baby
There are these terms now that it's just one person,
some plastic surgeon or some marketing person decides they're gonna
get in the magazines, they're gonna get on Instagram. They're
gonna come up with something that no one else has done.
Not that they're coming up with a new procedure, but
they're coming up with a new name for a procedure.

(12:46):
So the latest craze on Instagram apparently is the baby rhinoplasty.
Doesn't that sound good? It minimizes the operation of a
rhinoplasty to why do you think they minimize it to
try and get you to have the procedure, because if
you think it's a smaller procedure, yeah, you're more likely
to sign up. And you know that's fairly disingenuous, I think.

(13:09):
So let's cut to the chase. What is a baby
RHINOPLASTI There's no such thing as a baby rhinoplasy. A
rhinoplasty is a nasal reshaping operation. It is known by
plastic surgeons as the most difficult procedure in all of
cosmetic surgery. It is considered to be the most difficult
because there are so many different variations in the appearance

(13:32):
of the nose. There are hundreds and hundreds of different
steps that I can take as a plastic surgeon to
do different things like narrow the bones or wide in
the bones, or or make your hump smaller, or make
your hump bigger, or narrow the tip or wide in
the tip, or narrow your nostrils, or work on the septum,

(13:53):
or make your breathing better, or or take a boxy
tip and make it narrower. And there's so many different
things that can happen with the nose. Every nose is
a little different, and a good plastic surgeon that does
a lot of rhinoplastason I do. I love doing Rhinoplastis
is my favorite operation because they are the most difficult
and they are the most creative of all procedures in

(14:14):
cosmetic surgery. So when I do an operation, it takes
about three hours to do a typical rhinoplasty, and I
hear these terms baby rhinoplasty to try and minimize the operation.
That is just crazy, because you've also heard of these
people having their nose done twice, three times, four times.
How about Michael Jackson, you know, six, seven times? Unverified

(14:35):
of course, but that's the word, you know. He wasn't
so nutty that he wanted his nose done over and
over again. It was that it wasn't done perfectly and
so he wanted more. And that's the typical story of
someone who has their nose done many, many times. It's
also the typical story of someone who might not be

(14:55):
that good at doing Rhinoplasty's now full disclaimer here that
every the honest plastic surgeon will tell you that the
redo rate of a rhinoplasty is somewhere between fifteen and
twenty percent. There was a plastic surgeon in San Francisco
who just passed away, Ron Gruber, who was a very
very good plastic surgeon. He only did rhinoplasts, and he

(15:16):
used to say to his patients that the rhinoplasty is
a two step operation. I'm gonna do it and then
we're gonna revise it in six or twelve months, you know,
and that's set the expectations of the operation because it's
such a common thing to have to redo something. Whether
it's a small thing. There can be a return of
a hump, or they can be a stitch that the

(15:36):
cartilage that was placed in the cartilage can pull through,
or they can be scarring or something. There's a lot
of different things that can happen in a rhinoplasty. So
when we use terms like the baby rhinoplasty to get
people to sign up, go on Instagram and say we're
gonna do a procedure. I saw something this week. Someone
said I was I had a baby rhinoplasty and I

(15:57):
was at a party an hour later. Now that's nuts.
You know that as a consumer that if it's too
good to be true. It probably isn't It's ridiculous. So
aronoplasty is an operation that I usually do under general anesesia.
I make an incision between the nostrils. It's called the columella.
It's the little bit of skin between your nostrils, and

(16:19):
I lift up the skin of the nose like the
hood of a car, and I can see the cartilages
and bones of the nose. And you know, there's no
X ray that you can do beforehand. There's no cat
scan or MRI that can be done beforehand to show
what the cartilages are like. So often it's a surprise.
You know. We do the best we can by manually

(16:40):
examining the cartilages. I sit you down in a chair
and I feel the nose to get an idea what
the cartilages are like, what the bones are like. But
then it often when we lift up the skin, there
can be surprises and we have to be ready for anything.
I've seen many people who had cracked cartilages and you
could not tell that before surgery because they healed with

(17:03):
scar But once we open things up and can look
at the at the cartilages, we have to fix those
crack cartilages, because if not, there's going to be a
deformity after the rhyinoplasty. So a rhinoplasty very difficult operation.
Have you heard of the liquid rhinoplasty that's just injecting
filler like radius or huronic acid into the nose happens

(17:23):
to be one of the most dangerous things we can
do in plastic surgery. There are hundreds of cases of blindness.
I said it, blindness from injecting filler into the nose.
So it's a potentially dangerous area. When it is done,
your plastic surgeon should be using ultrasound now that's the
new thing, to make sure that the blood vessels, the
big blood vessels, are nowhere near the injection site. I

(17:47):
cringe when I hear that some nurses, physicians, assistants, dentists
doing this operation with filler, the liquid rhinoplasty, and they
haven't operated on the nose. They don't know where those
structures are. And I worry a lot about procedures like that.
Doesn't take a lot to cause a big, big problem.
So baby rhino plast I don't use those terms. It's

(18:09):
a rhino plasty. It's a serious operation. I do a
lot of those. Love to do those. I spend two
hours with you before the procedure, two full hours, an
hour in a consultation and an hour in a planning
session before surgery, and then we operate and it's usually
around a two to three hour procedure, depending on all
the different steps that have to be done. All right,

(18:31):

I've been saying it the last few weeks. I have

all sorts of things in my office, do filler and
boattox and take moles off and fix ear lobes and
things like that. And then when we do rhinoplasts and
facelifts and things like that, I do those at Manhattan
nin Ear or at the Surgeon Center at Robert with
Johnson or Cares in New Jersey. But I've also just
joined well by Messer. That is an incredible group of

(20:36):
a four ender chronologists and a couple internal medicine doctors
and a psychologist and a gynecologist, and some sports medicine
people and nutritionists. It's a great group. I am now
their plastic surgeon, so happy to be doing consults and
treating patients over at well by Meser. They're at one

bang for your buck procedures, Now, those are the procedures

(21:40):
that I think are, you know, the most effective procedure
for as little money as you want to spend. Now, look,
let's face it, cosmetic surgery is not an inexpensive field.
You know, even the least expensive procedures are are fairly expensive.
That's the nature of cosmetic surgery. Plastic surgery has trained

(22:00):
six years after a medical school, when you've got offices
on Park Avenue. You know, there are expenses and things,
and there's no such thing as an inexpensive plastic surgeon.
But there are procedures that you can have that are
very effective and improving your appearance, and then hopefully they
won't break the budget. So each year I kind of

(22:21):
look at what's out there, and things have changed around
a little bit, but certainly I think that the number
one procedure, the biggest bang for your buck procedure and
all of cosmetic surgery has got to be liposuction of
the neck and jos because in an hour and ten minutes,
an hour and fifteen minutes under local antesesia, or maybe

(22:41):
a little bit of sedation if you want, it can
be general antesesia. But it's so easy to do this.
I make a little incision under the chin and one
little incision under each ear and numb things up and
suction out as much as eight ounces of fat. It
will make you look like you've lost a lot of
weight without having lost anything at all except those eight
ounces of fat.

Uh.

And it is such a great procedure. You'll be back
at work. Well, this is one. I mean, you could
be back at work in a couple of days because
you'll feel okay. But you've got to wear scarves or
maybe that big COVID mask or or you know, a
big turtleneck if you want not to have that bruising.
Then you're gonna have to take a few days off,
maybe a week or two. So that's number one. The

(23:23):
second thing is always a surprise to people that I
tell this too, But it's removal of the things on
your face, whether it's a mole or some people walk
around with lots of seberria keratoses and skin tags and
things like that. Those things are distracting to other people.
You get used to looking at those because you kind
of cancel them out. But certainly in an hour, I

(23:46):
can shave off those sebria keratosis and trim those those
little skin tags and remove some moles, and and you
will look so much better, so much better, and sometimes
not always some of those things are covered by So
that's number two. Number three, I'm doing one of those
in a couple of weeks. A chin and plant. You know,
they're less common now than they were before because now

(24:10):
we have filler, and I can do a chin augmentation
with filler very easily, but chin implants in about forty minutes,
I can permanently, really permanently make your chin look so
much better, and it balanced us out your nose. Filler
around the mouth is the fourth hot procedure, and boy,
I'll tell you, I do a lot of filler, and
it makes such a big difference in those of you

(24:31):
who have never had filler, and you're in your sixties
or seventies, it is life changing. We can roll back
the clock by ten years sometimes. And the last one,
again a surprise for many people, but laser hair removal
of the of the hair on the lip and the
beard the chin. For women, particularly women that have polycystic

(24:53):
ovary syndrome and get hair on their face, it is
such a huge thing to not have to shave that hair.
Laser hairy moves is an easy, simple, straightforward and quick
way to remove that. I do that in my office
in Park Avenue also, so there you go. The list
for twenty twenty five a little bit different from lists
over the years, But these are the procedures that I

(25:15):
think are easy to do, relatively low risk, relatively inexpensive,
with a big impact on your appearance. You don't have
to go into the operating room, you don't have to
have a facelift, rhinoplastic breast augmentation. You can still get
a real boost to your appearance with these fairly simple procedures. Well,
