You know, it's almost summer, really, and I hope even
on a day like today, which had a UV index
of four, you know, a cloud cover you still need
sunscreen this time of year, all right, So I'm a
board certified plastic surgeon. I talk about wrinkles, I talk
about saggy breast. I talk about facelifts and what's called

blepharoplasts that's an eyelid lift, and ryanoplasts that's a nasal.

Re shaping operation.

I talk about fat on the hips and how I
remove it with lippos suction or maybe a per tuber
and belly after pregnancy, well that's a tummy tuck. These
are the things that we talk about on this show.

Effects of retinoids. That's a lot of words, isn't it.

Well, so many people use vitamin A use retina at
night or maybe my product which is called Nighttime, or
maybe some of the Johnson and Johnson products which have
retinol in them. If you're using a vitamin A, you
need to do.

Speaker 3 (03:37):
A couple things.

Speaker 4 (03:38):
First of all, you need to use sunscreen because it
sensitizes your skin, as do the alphydroxy acids, the glycolic
acid substances that you put on your skin. You need
to use a sunscreen. You can get an easier sunburn
if you do not. But beyond that, they irritate. The retinoids.
Vitamin A of any type irritate your skin. And a

(03:59):
lot of you who use retine know that your skin
can get a little red and a little irritated. And
if you start in right away and don't let your
skin acclimate to the vitamin A, then it can get
pretty raw. But the antidote for that is niacinamide vitamin
B three and pantothenic acid vitamin B five. They make

(04:20):
the irritation from the retinoids easier to handle and in
fact decrease the irritation. So their bookends daytime in the morning,
nighttime at night. That's my skincare program. We're giving away
bottles of daytime today. So give me a call. You
have to have a good call though, Yeah, I mean
you can't just call and say send me the stuff now.

if you were on Instagram or Facebook or just checked
out the news, you probably heard about Kylie jen and
Barbara Corcoran. They both came out of the closet with

their cosmetic surgery. So we're going to talk about transparency
and cosmetic surgery, particularly about them, and we're also going
to talk about there's there's six different batcha linum toxins.
Now you call them botox, right, but that's the that
was the first one. There's six of them. And the
question that I'm asked all the time by my patients

is does it make a difference which one I use?

Why aren't using botox? Isn't that the best one?

Speaker 4 (05:35):
We're going to answer that because there was a study
this week that actually did answer that question. So you'll
want to listen to this if you're one of the
eight million people in the United States that have used botox.
And we're also going to talk about combining technologies. There's
a combination treatment using both the laser and all therah

you're using the gl one p inhibitors, you know what
I'm talking about. They are things like ozepic and zip bound.

You know, if you're successful, and hopefully you will be,
you're gonna lose weight. And when you lose weight, you've
got to come and see You don't have to come
and see me, but so many of you do because
you've got that extra skinnier neck or you get that
what's called a nozempic face, you know, kind of a
sunken in face. You know, it doesn't happen to everybody, no,
but it does happen to some people. And if you've

lost fifty pounds or so, you're you're gonna have extra
skin of your belly, just like delivering after a pregnancy
or just losing weight without the GLP one drugs, without
zep bound and ozempic, you lose weight rapidly. You wind
up having extra skin that a plastic surgeon can help

you with with facelifts and breastlifts and tummy tucks. And
so that's what I do in the practice, and I
inject botox and filler and things like that. So if

but we do have a phone call from Sodra. Sondra,
what can I do for you? What's your wrinkle?

Speaker 7 (10:43):
Hi, Doctor Perry. I remember a long time ago you
spoke about a non invasive cosmetic prestigere quilled all there.
So I was wondering if you could talk about that again.
Does it work, how long does it hurt? Who benefits?
Because if that's like something ONVA, maybe it's a good
thing to try.

Speaker 4 (11:03):
Well it is so as a matter of fact, it's
going to be one of my topics later. But we
can get to that right now because ol Farrah. Okay,
So here's the thing. Plastic surgeons have a lot of
different ways to stimulate healing. And we stimulate healing. And
why would we stimulate healing. We have to create a
wound in order to stimulate healing. But we do that

(11:24):
because the healing process shrinks tissue and builds collagen. Now, plastic,
here's the secret. Plastic surgeons use that euphemism. We call
it collagen. It is collagen, but it's also scar. But
we don't If I told you I was going to
create scar in your face, you'd never come to me, right,
But if I told you I would generate collagen, well that's.

Speaker 3 (11:45):
A good thing, right.

Speaker 4 (11:47):
So there's different ways we do that, Sondra, And it's
almost like a Saturday Night Live skit. You know, how
many ways can we hurt you? We can hurt you
with a laser. Oh sounds bad. I'm not a good
marketer right when I talk about hurting. But a laser
is a machine that intensifies one particular color of light

(12:08):
and it passes through the skin, generates a little heat
injury and stimulates you to shrink the skin and generate collagen.
Althra does something very similar, but instead of using light,
it uses sound energy. Now you know that sound has energy.
Although you might not think about it. If you stood
next to a drummer right in a parade or something

(12:30):
like that, your clothes might vibrate. That's because the drumming
on that the sound of that drum, the waves travel
through the air and actually can cause movement in your clothing.

Speaker 3 (12:42):
Right, So there you go.

Speaker 4 (12:43):
It can do that, and we can actually focus that,
we can intensify it and focus it, but put it
in a frequency a sound that you can't hear. That's
called ultrasound. So that's how ultra shape. Sorry, the Altherra
rather ultra shape is different technique that uses ultrasound. Altherra
uses that high energy focused ultrasound, and it like a

(13:08):
magnifying glass, kind of takes that light and focuses it
at a pinpoint. You can burn things in the sun. Well.
Altherra focuses the energy of almost like a train, the
sound of a train going past you to one and
a half three and four and a half millimeters beneath
the skin, and it creates a little bit of an injury,

(13:30):
and it doesn't hurt the skin at all, So there's
no downtime, you don't have any.

Speaker 3 (13:35):
You can go to. You can be on TV that
very same day, Sondra.

Speaker 4 (13:41):
But what it does is it actually burns the tissue
a little bit, so it hurts a little bit. I've
had it four times. I'm well overdue for another treatment.
I do it on the cheeks to lift the jows
a little bit. I do it on the neck to
decrease the crepiness of the neck. I do it on
the far head to lift the brow. I do it

(14:03):
on the top of the neck called the day, rather
the bottom of the neck called the descola tage.

Speaker 3 (14:09):
It's the top of the chest.

Speaker 4 (14:10):
It's the only thing that we can really do for
those lines that women get. And now's the time of
the year. You know, you're putting on your bathing suit
for the first time and you see the vertical line
in the middle and those V shaped lines of the
upper part of your chest. When I use al thera,
it really is the coolest thing. It's almost like ironing
a shirt because we go over the area sometimes I'll

(14:31):
combine it with injecting a little bit of filler into
those wrinkles. But I do that and many women, and
we do it maybe once a year, maybe once every
two years. The effect is not long lasting. I mean
it's long if you consider once two years long. It's
not gonna last ten years. It's not gonna last five years,
so you have to redo it. But it is the

(14:52):
only thing that can help those lines. So what I
was going to talk about today is there was a
study that came out that used all therah along with
the fractionated carbon dioxide laser in the very same treatment day,
and they do kind of different things. Alterra will kind
of lift the tissue and the laser will decrease some

(15:15):
wrinkles and decrease splotchy pigmentation. And it is safe to
do both in the same day. You know, I'll give
you a little bit of that brand new pain medicine.
It's called Jernas. You can't even pronounce that jo your
ana x V. I believe it's it's a strange word,
but it's the super tyleomol and it's a non narcotic,

(15:37):
So you can take it and drive into the office
and then you can have those procedures and al therah. Again,
it's one of the few procedures that I've had.

Speaker 3 (15:47):
It works. It's a good procedure. It's not a facelift.

Speaker 4 (15:50):
It's if you're expecting facelift results, No, you're not going
to get that, but you will get some results. And
about twenty percent of people say wow, and about seventy
percent of people say, yeah, it's good.

Speaker 3 (16:01):
It's better.

Speaker 4 (16:02):
And about ten percent of people say, I don't think
it worked at all. And that's the honest answer in
those people. If I do it a second time, usually
we'll convert those to that seventy percent group where you
look in the mirror and.

Speaker 3 (16:15):
Say, yeah, it's helpful. I look better. So that's your story.

Speaker 7 (16:19):
Sondra very interesting, like a kind of coin that could
have burn you skin that laser thing or not?

Speaker 4 (16:28):
Really, well, are you talking laser or altherra. So the altherra,
it's so unlikely I have had. I've done thousands of
people now and I've had maybe one or two blisters.
Believe it or not, you can get that. It's possible.
But it's one of the safest procedures that we have
in plastic surgery and one of the few that has

(16:50):
zero downtime now with zero time downtime. Also, it's not
an immediate effect. We get some benefit right away, but
it takes two months to see the full effect of
al Farrah, and it lasts anywhere from one to two
years the effect. So that's your story.

Speaker 7 (17:05):
Okay, it's a little painful, not a lot, just a
little bit.

Speaker 4 (17:10):
You know, women do much better than men, it's true,
and and everybody does better than me. I'm a pretty
bad patient. I scream and the words that come out
of my mouth are not pretty when I have procedures
like that. But most of my women patients, they say,
not bad at all. And some women say, oh, it's
like a massage, and I look at them and my

(17:31):
nurse laughs, Yeah, remembers how I tore the table paper
having my procedures.

Speaker 3 (17:39):
Yeah.

Speaker 4 (17:39):
Anyway, So, but they're good procedures there. I do those
in my Manhattan office.

to be my third topic, but we're going to get
to the topic of Kai Jenner. She came out of
the closet with her breast augmentation. And after she did that,
Barbara Corcoran said, you know what, all these young guys,
these young celebrities are coming out of the closet, I

can too. So she talked about her cosmetic surgery. This week,
when we come back from our break, we're going to

All right, so let's talk about Kylie Jenner. Kylie Jenner,
you know she's out there in the Kardashian family. You know,
everybody knows the Kardashians, right. She came out this week
and gave very specific information about her breast augmentation. I

(20:36):
guess someone had asked her online and she went ahead
and said, well, sure, I've had a breast augmentation. And
I think she said she has four hundred and forty
five cc moderate profile dual plane breast implants in there.

Speaker 3 (20:53):
And there you go.

Speaker 4 (20:54):
Now now if you look at her now, first of all,
thank you Kylie for coming out and saying that you've
had cosmetic surgery, because too many people hide it, and
it's so obvious. If you're an A cup one day
and a C or D cup a couple of weeks later,
either you know you had some weird mutation or you
had cosmetic surgery. So you know everybody knows, so you

(21:17):
might as well come out and say it.

Speaker 3 (21:19):
And she did.

Speaker 4 (21:20):
You know, Joan Rivers was the person that really popularized
talking about cosmetic surgery, and good, now I look at
Kylie Jenner, and I think her four hundred and forty
five cc implants are way too big for a petit woman.
You know, she's into a d cup now. And my
bet just wait long enough and Kylie five ten years
down the line will take those implants out, or maybe

(21:42):
fifteen years and put much smaller implants. I would have
put two fifty to three hundreds in her, but hey,
that's splitting hair. So so then Barbara Corkoran. We all
know Barbara from Shark Tank, right, you know, and I
look at her. She's the realtor. And she came out
this week and on threads that's I guess Facebook's X

(22:04):
or whatever it is. So she came out. She's seventy
six years old, and she said, I've had a lot
of procedures. I don't just wake up like this, which
is pretty funny.

Speaker 3 (22:14):
Good for her.

Speaker 4 (22:15):
She says she's had an eyelid lift, a neck lift.
She says she gets ear filler four times a year.
Maybe she listens to this broadcast. I've talked about putting
filler in the ear, and I think I was one
of the first that talked about it, because you know,
people don't talk about that, but I do it almost
every day where women come in with you for getting

(22:39):
filler around the mouth, and I say, do you want
a little in your ear lobes because your ear lobes
are sort of wrinkled and involuted, and I'm sure your
ear rings are turning to the side, So we put
a little bit of filler in and the ear rings
sit nicely. Again, Well, she's had that. She says, she
gets it four times a year, which is a little,
you know, a little funny. Okay, what can I say?
She says she has the fractionated CO two laser on

(23:03):
her face every year, once a year. And that's interesting, now,
you know I do that on women. It usually lasts
a lot longer than a year. But it depends on
how deep you go with a fractionated CO two laser.
So if we go very superficially, well yeah, it probably
will last just a year. If I go deep, it

(23:24):
could last many, many, many years. And let's see what
else does she have? She says she's had three facelifts,
a lower eyelid, skin pinch filler four times a year,
also professional teeth whitening, all sorts of things. Well, and
she and really it's very good because people then look
at her and say, Okay, you know you're seventy six

(23:45):
years old. You're aging gracefully with cosmetic surgery, which is great.
And there's no way to look as good as Barbara
looks at seventy six years old without having cosmetic surgery.
She's done a service to those people people out there
who want to have cosmetic surgery and you know, are
maybe afraid, they're afraid of the results, afraid of what

(24:08):
people might say. I think these things are good. Now
there's critics everywhere, both for Barbara and for Kylie Jenner,
and they say, oh, my goodness, we are we are
normalizing cosmetic surgery, and why not? Of course, why not?
You know, we wear nice clothing. If you're if you
have the financial means to have cosmetic surgery, if you're healthy,

(24:31):
then why not. Right, That's what I say. And and

But a lot of patients want to keep it really quiet.
I guess that's that's you know, your option. But there's
