fires in the Hamptons, but I you know, they're not
that bad actually from my perspective. Anyway, I saw the smoke.

Yeah yeah, you can hear that in my voice because
I was breathing it. But anyway, we're gonna be here
broadcasting live and taking your phone calls and we're gonna
talk all about you and about your wrinkles and about
maybe small breasts and droopy breasts.

That is going to be the topic tonight because yeah,
I did one of those procedures. This week, we're going
to talk all about liposuction of the neck and jowls
and the lower facelift and maybe some less invasive methods
to make you and help you look better. So the

I always talk about the biggest bang for your buck
procedures in cosmetic surgery, the ones that give you the
most result for.

The least cost, and also the least risk.

And on the top of my list, probably for all
twenty years I've been on this show, is liposuction of
the neck and Jols you can call it liposuction, it's
really lipos suction. And that is a procedure that can
through little tiny incisions underneath your chin and a little
incision under each ear, I can break up the fat

(05:00):
and I can suction it out, and I've suctioned out
as much as eight ounces of fat from the neck. Now,
you can be rail thin but have a fat neck,
and people look at you, you're in clothes and people think,
you know, you're kind of chunky, right, a little overweight.
And when I remove that fat it looks like you've
lost ten pounds, fifteen pounds, sometimes even more just tightening

(05:24):
the skin of your neck. And I'm going to talk
to you about how that can occur without ever removing
skin of the neck. So we're going to talk all
about liposuction of the neck and jows, and we're also
going to talk about the alternative methods of rejuvenating your neck.
So when we think of a facelift, it's such a
poorly worded name, isn't it.

Speaker 6 (05:45):
It's a procedure, It's a face. What's the face? Is it?
The people say, is it the upper face? Is it?
The middle face? Is it?

Speaker 3 (05:51):
The lower face? Is it that everything? If you have
a facelift, do you have to address the brows? So, really,
a facelift is classically a cheek lift and a neck lift.
But a lot of people don't need anything in their cheeks,
or don't want anything in their cheeks because when we

(06:11):
lift the cheeks, that lengthens the procedure significantly, and particularly
in men, men don't do too well with a facelift.

Speaker 6 (06:19):
They really don't.

Speaker 3 (06:19):
You listen to all the hype you want, but the
truth is men don't like facelifts. Because they don't wear makeup.
And also it puts hair bearing skin in the ear.
So if we can do something for men in particular
without making incisions in the cheek, that's a good thing.

So if we just address.

The neck, well we're not going to lift the jowls,
but we can suction the jowls through a little incision
in the neck, and we can also remove fat and
we can tighten the bands of the neck. So when

I'll talk to you about the of doing that, and

as a plastic surgeon. Yes, I'm a real plastic surgeon.
I spend my days during the week in my office
at eighty fifth in Park it's my new office in
Manhattan and also my office in Somerset, New Jersey. How

(08:46):
can I be at two places at once. Well, I
divide it up. But we do the laser procedures and
minor surgery in the New York office, and other procedures,
bigger procedures we do either in the hospital in Manhattan
nine or at Surgis Center. So that's how I do it.

help you. So the biggest bang for your buck procedure

(09:09):
in all of cosmetic surgery, it's lip a suction of
the neck and jous. And the reason I call it
that is because through a relatively short procedure, it takes
anywhere from an hour to maybe an hour and a half.
I can do this under local antesesia. I can do
it under straight local anesthesia. That means we just numb
up your neck. We don't have to actually go even

(09:32):
to the hospital. For some people, it depends. I mean,
if you've got some medical issues, or if we're doing
other procedures like tightening the bands in the neck, then
we'll do it in the hospital or in a surge
of center. But if it's just straight local anesthesia, and
I know that you're healthy, you don't have any medical issues.
We cannot often do it right in the office in

(09:52):
New York or in New Jersey. So here's how it's done. So, actually,
let's let's back up a bit. When you age, when
you ag, we tend to put fat where we don't
want it. We want it, we take it away from
where we do want it.

Speaker 6 (10:06):
Right.

Speaker 3 (10:06):
Women tend to lose fat in their breasts, but put
fat on in their neck, around their hips, their thighs, right,
you know. So it's one of the ironies of life.
You lose fat where you want it, you gain fat
where you don't want it. And so fat removal is
a big, big business. It's surgery. There are surgical techniques

(10:28):
and then there are non surgical techniques. Now I do
a bunch of these different techniques, and it depends on you.
It depends on what you want and really what you need.
So let's say you're a sixty year old woman that
comes into my office and you've got a little drooping
skin of your face but not too much. You've got

(10:48):
some early jewels, you've got the turkey gobbler, you've got
extra fat in your neck and you really can't see
the bands. Now, I'll examine you, and i'll feel you,
and I'll do the best I can to determine the
position of those future bands, because if I remove the
fat in your neck, it's very likely we're going to

(11:11):
then see the bands because the fat is hiding the bands.
So the plastic surgeon needs to really examine you very
carefully and make sure that if liposuction is performed, we
don't expose bands of the neck and we don't get
too much drooping of the skin.

Speaker 6 (11:28):
So how do we do that.

Speaker 3 (11:30):
It is by the experience of the plastic surgeon. There's
no X ray that we can perform. We won't want
to do that. We can use a little bit of
ultrasound sometimes to see the position of the muscles and
where the bands are, but most of the time the
plastic surgeons can just examine you and make that determination.
So if you are thirty and you have a lot

(11:50):
of fat in your neck, well then it's a straightforward
decision to perform liposuction. And the liposuction involves about a
quarter inch decision right underneath the chin, and about even
less even a shorter incision under each year, and I
put a large amount of very dilute lightocane that's a
local anesthetic into the area of your neck and it's

(12:12):
called the sub mental area, the area underneath the gin.
And also I go into the lower part of your
cheeks with a little bit of lytocane and then I
introduce these little tubes we call those canula, and I
break up the fat of the neck and I suction out.
There is absolutely no discomfort at all during the procedure.

Speaker 6 (12:32):
Once I numb you up. Numbing you up, you know,
I go slowly.

Speaker 3 (12:36):
I'm very good at it, and if you're in the hospital,
most people do get a little bit of sedation. We
zonk you during the procedure while I numb you up
that's a technical term. And then we just do it.
It feels like a massage. And afterwards a lot of
people think, well, it's going to hurt a lot, right,
I've put a steel instrument underneath your skin, and I've

(12:56):
broken up the fat and I've suctioned it out, and
it doesn't hurt much at all.

Speaker 6 (13:01):
In fact, the.

Speaker 3 (13:02):
Person I did this week only tile, only tile and all.
And that's the average person. I can't even remember the
last time, I had to prescribe anything more potent than
just tile and all for liposuction, because if we don't
bang into the underlying muscle, if we don't bang into
the bone or underlying structures of the neck, it really

(13:23):
should not hurt. So afterwards you look terrible, though, you
really do. You look so swollen and so bruised. You're
not going to go out for a few days. But
the nice thing about liposuction of the neck and jows
is that it doesn't the bruising doesn't go above the
level of your jawline. So you can use a turtleneck,

(13:44):
you can use a scarf, and you'll feel just fine.
So three or four days after the procedure, you could
go out to a restaurant or something like that. You're
not going to be the center of attention at a
wedding three or four days later. You'll wait about maybe
two weeks or so before you're going to be photographed.
And if it's your child's or your wedding, well, you
sure want to wait even longer before you schedule something important,

(14:08):
because if there is a prolonged swelling, if there is
prolonged bruising, or if you see a lump or something
like that. Boy, you don't want to go to the
wedding the next week. So you have to be careful
and use common sense when you plan these things. So
now if you are that's the thirty five year old,
the forty year old, what have you? That sixty year
old woman? And maybe I decide there are some bands

(14:31):
in the neck that if I remove the fat, we're
going to expose those bands. And those are those Catherine
Hepburn bands, remember her, of course you do. They go
down from your chin all the way down to the
collar bone, and they can be quite stark. They're hidden
often by thick skin when you're younger. But as the
skin thins as you get older, and it thins at

(14:54):
a rate of one percent per year, well, we begin
to expose the underlying structures and other un lying bands
of the neck. You see the the veins of the neck,
you see the muscles, and so we have to be
cognizant of that. So what do we do in that situation.
It's not just a quarter inch decision, and this is

(15:15):
usually not under straight local anteices. When we extend this
to the platisma plasty, that's the name of the operation.
So it's the platisma p la platisma plasty. And when
we do that often then we'll do this under intravenous sedation.
I rarely do this under general anesthesia, and the reason

(15:37):
is you don't have to.

Speaker 6 (15:39):
It's just not a painful procedure.

Speaker 3 (15:41):
But also we don't want you to get nauseated, and
general anesesua causes nausea in about twenty percent of people.
So the drugs at the anithesiologists at Manhattan i in
ere at Robert Wood Johnson, or at Care Surger Center
in New Jersey, the drugs that the antithesiologists choose are
ones that specifically do not cause nausea, things like propofol,

(16:02):
which all of you who have had colonoscopies, you've had propofile.
It's a very smooth drug and it sedates you and
it makes it so that you kind of you know,
we say okay, we're going to start the procedure and
then we say all right, it's all over.

Speaker 6 (16:14):
We're going to the recovery room, and you say, did
I have it? Yep. That's how it is with propofole.

Speaker 3 (16:18):
We give you a little verse at also, which is
like valiant but we stay away from the drugs that
can cause nausea, things like narcotics and certainly general anesthesia,
so sedation works very well.

Speaker 6 (16:30):
Also, we don't want a tube in.

Speaker 3 (16:31):
Your mouth, a breathing tube if we are going to
be working in your neck. It's much simpler, much easier
to not have to work around a breathing tube.

Speaker 6 (16:42):
So this is a.

Speaker 3 (16:42):
Procedure that I do under sedation or just straight local anesthesia.
If I've operated on you before and I know you,
we can often just do this under a local anesthesia.
And the advantage of that is, well, it saves you money,
of course, because you don't have to pay for an antithesiologist,
or if we do it in the office, you don't
have to pay for the facility.

Speaker 6 (17:02):
So that's good.

Speaker 3 (17:04):
But also it just makes it much much simpler. And
there's certainly a lowering of blood pressure or a rather
a less chance of getting an elevated blood pressure if
we're not in the hospital, if we're not in the
operating room, if we're just in the office facility, So
there's advantages to that. So what we do in a

(17:26):
platisma plasty is I make that decision instead of a
quarter inch underneath your chin, it's about an inch or so,
maybe even a little bit longer. And I lift up
the skin of your neck kind of like if you
were to lift up your shirt to look at your chest.
I'm looking that way, and I've got to remove or
reduce the connections between the skin and the underlying tissue.

(17:51):
And when I move the muscles together. That's kind of
Let's say you had a shirt underneath your sweater and
you lift it up the sweater and you're looking down
out at the shirt. It's a button down shirt and
it's unbuttoned. You need to button those buttons to make
the shirt continuous. That's kind of like the platisma plastic
I take stitches and I go from the chin down

(18:13):
to almost your collar bone and then back up again.
And as I tighten those muscles, interestingly, they hug the
contour of your neck even more. They don't bowstring, they
hug the contour. And when I come back from the break,
I'm going to tell you the math secret that plastic
surgeons use to make your neck look better in neck

(18:35):
lippo suction. It's really kind of interesting. Tell your kids
that are in tenth grade geometry to listen. This is

have a geometry lesson here now, all right, So we're
talking about the biggest bang for your buck procedure and
all of cosmetic surgery, liposuction of the neck and jowls.
It's a procedure that I do a lot of. In fact,
I wrote the chapter on this subject. If you go

in the topic liposuction of the neck and jowls, you'll
actually see my chapter. It's written four plastic surgeons, but
you can read it and you can see some of
the pictures of my patients in that chapter on that chapter.

This is a procedure I've done so many of and
I love doing the procedure because it is such a

(20:47):
high satisfaction procedure. The risks are very low in my hands.
I mean, you know I shouldn't say that, right, but
anything can happen to a plastic surgeon or a patient.

Speaker 6 (20:56):
But certainly the.

Speaker 3 (20:57):
Risks are very low, and that's good. I've done so
many of them for over three decades. But it's interesting
if you think about it. So I'm removing the fat
in the neck and you say, well, doesn't the skin
just hang? Doesn't it just look looser because now I've
deflated the neck. Here's why that usually doesn't happen. It

(21:21):
usually doesn't happen. For one thing, we're tightening the platisma muscle.
That's what I told you about. If we tighten that muscle,
and sometimes by the way, I have to separate the
muscles and remove some fat.

Speaker 6 (21:31):
From underneath the muscle.

Speaker 3 (21:33):
That's often missed by people who are not particularly good
at this operation. They forget that there can be fat
underneath the muscle, not just over it. And if they
don't remove that fat afterwards, you're still gonna have kind
of a chunky neck. So we need to really understand
what we're doing here. But when we tighten that muscle
of the neck, it hugs the contour of the neck

(21:55):
progressively tighter and tighter, and the skin follows that muscle.
The skin will drape over the muscle. And here's where
tenth grade geometry comes in. Remember the Pythagarean theorem. Oh,
Noah remembers that in tenth grade. I think he just
said he took tenth grade math. In twelfth grade, I remember,
I got a ninety seven. And okay, we don't want

(22:18):
to hear about my grades in geometry. But I did
very well in did very well. And who knew I'd
be using the theory the Pythagorean theorem years and years later,
and you forget about that. Remember that a square plus
b squared equals us c square the hypotanus pipot news
of the triangle. That's the distance between you know, on

(22:42):
your neck, if you've got a fat neck.

Speaker 6 (22:44):
It's the high pot news of a triangle.

Speaker 3 (22:45):
And when we removed the fat of the neck, we've
got the two sides of the triangle, and the distance,
the distance of the two sides of the trigle is
actually longer than the high pot news so that means yeah,
and there's a point to this. When I remove the
fat from your neck, the skin redrapes over a longer
distance and looks tighter. That doesn't feel any tighter, but

(23:09):
it looks tighter because it's got to redrape over a
longer distance. And so even if I haven't removed any
skin from the neck, if I do this procedure, it
will usually look tighter. Now, I say usually because there's
exceptions to the rule. You might have so much extra
skin in your neck that it really does need to
be tightened. And then I would tell you in advance, oh, well,

(23:31):
we're gonna have an issue here. We'll have to do
a cheek lift to remove skin from the upper part
of your face and tuck the neck skin up. But
in most people, and I've done this in people as
old as the late sixties, I haven't done it in
people and women in their seventies. I've done men in

(23:51):
their seventies. And the reason is we make compromises. We
often make compromises in plastic surgery, and a lot of
just say, well, I'm never going to have that face left,
so let's see what we can do, and I'll accept
a little bit of hanging skin, and sometimes we might
have to go back a year later or whatever and
actually remove a little bit of skin from the middle

(24:12):
of the neck. A direct excision of skin a rare operation,
but it sometimes is necessary. So liposuction of the neck
and jou is one of the big bang for your
buck procedures. But you may say, well, wait a minute,
aren't there's other procedures, there's non invasive procedures. How about lasers, Well,
the truth is we have to be very very careful

(24:32):
about the lasers in the neck. The lasers can tighten
the skin of the cheeks, but if we go aggressively
in the neck, we can get scarring. How about a
thera all therah all therapy. That's one of the procedures
I love to do. And yes, that does tighten the
skin of the neck, and it also will remove a
little bit of fat. But it's a modest improvement and

(24:54):
really only about about maybe two thirds of people say, Okay,
it's significantly tighter, but alterra is an option. What about
cool sculpting? Cool sculpting, my opinion, two millimeters of fat
reduction not enough to make a big, big difference. How
about that, Kay, Bella. We talked about that last week.

(25:15):
That's injecting chemicals. Not wild about that. I think it's
got a bunch of complications I won't want to see
you have, and it's also a modest result.

Speaker 6 (25:24):
So there you go.

Speaker 3 (25:25):
In the end, it's back to surgery, and the surgery
should have a very low complication rate.

Speaker 6 (25:31):
Lipos suction of the deck and.
