upper Line Health, tell us alittle bit about yourself. Husband to my
wife and have four little kids fourto nine, and started upper Line six
years ago really with the goal oftransforming specialty care, but also sort of
a passion and desire to have awrite a work environment where people really feel

fulfilled and valued and learned a lotof that from my mentors. That's kind
of the driving force behind what we'redoing. What is your business about?
Our goal is to serve patients bytransforming specialty care towards value based care.
Value based care is changing the incentivesand the goals of a healthcare system,
both for providers to think more holisticallyabout patients, but also for our patients
to receive also more holistic care.And so there's a lot of ibased care.

It's happening on the primary care side, but we also know there's more
specialists in the healthcare system than primarycare docs, and so we want to
start tapping into that and providing thesame sort of models on the specialty side.
And so what we know is thatthe SACER patient is with different chronic
conditions, the more they see specialists. So patients that really are the highest
need of needing care coordination oftentimes getthe least care coordination. Our models acknowledging

that, so we want to builda model around where they are and meet
them where they are. We havea phrase that we always say is just
