talk about LDL cholesterol, that notorious bad cholesterol that's been
blamed for clogging arteries and causing heart disease. But here's
what you need to know. LDL isn't a villain always.
It's a complex molecule with a critical role in your body.

The problem isn't LDL itself. It's when LDL goes rogue
and it builds up in your arteries and forms plaque.
And until now we've been treating it blindly, throwing statins
at the problem without truly understanding how LDL works. But
thanks to a groundbreaking NIH study, we're finally cracking the code.

Using Nobel Prize winning technology, scientists have captured LDL in action,
specifically how it binds to its receptor and the liver.
The discovery isn't just a scientific breakthrough, I've considered a
game changer for how we may manage cholesterol and prevent
heart disease. So in this hour, we're going to break

enemy we've been led to believe. The NIH breakthrough that
maybe rewriting the cholesterol playbook and what the cholesterol numbers

to ditch that cholesterol is just a number thought and
let's block your body's ability to heal itself. So ladies

and gentlemen, today let's put cholesterol on tryle and maybe
the verdict may shock you that for about fifty years,
medicine has treated LDL like a common thief, blindly thrown
in statins at it without understanding as MO. But now,
thanks to this NIH team that's armed with a Nobel
(04:18):
Prize winning technology, we've finally got our smoking gun. It's
not just science, it's dietary story that with your arteries
at the heart of this. So picture LDL cholesterol as
this master cat burglar. It slips into your bloodstream wearing
(04:38):
a protein disguise known as APO B one hundred, and
it picks the locks to your liver's LDL receptors and
it empties your arteries of their elasticity or their vitality.
But until now we only had this blurry security footage
of the heist. But now with this this technology GUS
(05:00):
cryo electron microscope, it's got almost like a four K
surveillance footage. So the NIH scientists they captured the exact
moment where LDL that's the bad cholesterol, binds to its
receptor known as the LDL R and that's at the
very small atomic resolution. And so what we discovered was

that the family high cholesterol genes, the mutations, they act
like the greasy fingers. The grease fingers where LDL can't
get a grip to be cleared or taken out the bloodstream.
So what Statin's been doing is it boosts these receptor numbers,
but it doesn't necessarily fix those broken handshakes. So this

is like discovering a bank vault that has a secret
weak spot that's one that twenty eight million Americans on
statins could be exploiting all along. So I think this
changes quite a bit, changes everything. We're not just lowering
LDL now, we're out smarting it at the structural level.

So maybe it's going to be through fixes for that
family history, or I would say that the era of
the one size fits all statins is over with, so
as now using your diet in smart supplementation. It became
the most sophisticated cholesterol drug on earth. So if we

back it up and start with the basics of cholesterol,
you know, cholesterol isn't the villain we've been led to believe.
It's actually a vital molecule that keeps your body running.
If you think of it as like a building block
for cell membranes and also for hormones like estrogen and testosterone,
(06:54):
and even for vitamin D. Cholesterol is very important, but
the which is not all cholesterol is created equally, So
we talk about the good, the bad, and the ugly cholesterols.
We're talking about HDL, the high density lipoprotein. These are
the good cholesterol that scavenges excess cholesterol and takes it
(07:16):
back to the liver to eliminate or for disposal. The LDL,
the low density lipoprotein, is the bad cholesterol that delivers
cholesterol to cells that can build up in the arteries
if the levels are left too high. And then another
one that's not really known is what's called VLDL. It's
(07:37):
very low density lipoprotein, and this is that precursor to LDL,
and it carries triglycerides in cholesterol themselves. So the problem
isn't cholesterol itself, it's when LDL goes rogue. These LDL
particles they oxidize, they stick to the walls of the
arteries and they form plaque, and over time this plaque
(08:00):
narrows the arteries and it leads to heart attacks and strokes.
So that's why LDL has been public enemy number one
for cardiovascular health. So if we're diving into this groundbreaking study,
the NIH study. It's again, it's rewriting the cholesterol playbook
for the first time. Scientists use this cryo electron microscope
(08:24):
to capture LDL in action, specifically how it binds to
the receptors, those LDL rs receptors in the liver. And
this process is critical because it's how LDL gets cleared
from the bloodstream. So the key findings on this study
were that the LDL in that receptor handshake. Researchers visualize
(08:47):
the exact moment, that exact molecular interaction between the LDL
and its receptor. So if you think of it like
a lock and key, the receptor is the lock. In
the LDL the low density lipoprotein, that cholesterol is the
key for that lock. So these mutations in our genes,
(09:10):
the family history of high cholesterol very common, and the
mutation in that LDL the receptor gene, disrupts this handshake
and it causes the LDL to build up in the
blood stream. And then this is where statins come up.
Short statins increase that number of the LDL receptors, but
(09:32):
they don't fix the broken ones. So this explains why
some patients that are on statins still have high LDL
levels despite getting the statin treatment. So the discovery is
important to note here because by understanding the ldl's structure
and behavior, we can develop targeted therapies that fix that
(09:56):
root cause of the high cholesterol and not just mask
those symptoms. So if we're looking at what's next for
the breakthrough in cholesterol, we got to look at things
like the personalized medicine, the genetic testing that could identify
these receptor mutations, allowing doctors to tailor treatments for individual patients.

Then there may be some new medications on the line
that could stabilize the ldl's structure or enhance the binding
to the LDO, making cholesterol clearance more efficient for the body.
So this isn't just about lowering cholesterol. It's about understanding
and at the small molecular level and using the knowledge
to save lives. So cholesterol is not the enemy. It's

just a misunderstood molecule that plays a critical role in

We've been talking about a new breakthrough for cholesterol in
how it might be rewriting the rule book on it.
So it's a study by the NIH, and I don't
think it's just a breakthrough, it might be a wake
up call. For decades we've been this whack a mole
(13:01):
with LDLs using statins while ignoring the problem, which is
studying that. The study revealed that these broken handshakes between
cholesterol and your liver receptors are at fault. And of
course what we've been saying for years is that your
grocery list might be the ultimate fix. So the study
by the NIH revealed that LDL they're binding mechanism has
(13:26):
been looked at like never before. These familiar like the
hypercholesterol mutations that are passed down from in your genes.
They're like grease door knobs, and that LDL just can't
get a grip. So these statins they install more doors,
more receptors, but they can't fix that slippery handle. So
(13:47):
no wonder about thirty percent of the statin users still
battle high LDL levels. So maybe Statin's place will be
better understood and maybe more effective from here on out.
So this is something to demonize cholesterol. It's about hacking
its code as for years, your diet is the most

Well, thanks thanks for joining us today. I know you
got a busy schedule. So, Jim, you know cholesterol awareness.
It's a key component for everyone's heart healthy strategy. I
want you to explain why you think that is.
Speaker 6 (15:43):
Well. I think you know, first of all, people just
think of cholesterol may here, good is bad, right, HDLs
are good at LDLs are bad? Right, and your total
cholesterol number. And really science has.
Speaker 7 (15:55):
Come a long way. Right.
Speaker 6 (15:57):
First of all, we understand now that cholesterol isn't due
to eat, you know, necessarily a food desigh in fat.
It's probably more that your insulin resistant or pre diabetic
that you're making more lipids. But the reality is you're
you're you can either make good lipids or bad lipids,
(16:17):
you know, so it's bad actor lipids. And it's not
just LDL. It's how big is your LDL cholesterol? Is
it fluffy? If it's small, then it penetrates the innerlining
of your orgery, which as you said, it's a it's
a lab in that interlining of the artery, the glycoklyx,
(16:38):
it's that interlining. It's the LDL cholesterol can get in there.
Basically you're immune system attacks it and you start the
plaque process. And so how do we know that you're
likely for that? It's by looking at the different fractions
of cholesterol that can be a problem for your artery,
(17:00):
both on how stiff they are leading to the high
blood pressure, but then also if they're getting damaged. And so,
you know, cholesterol. You know people here that from ow
it's not that important. That really is important, but it's
important that you get a thorough lab to understand the
importance of it.
Speaker 2 (17:20):
Yeah, I agree with that. You know, people are saying, oh,
you know, cholesterol is just just a number, but you
need to focus on those individual numbers in particular that
that LDL and the HDL levels and it's affects so
many people, Jim, how many people are affected? By by
high cholesterol.
Speaker 6 (17:38):
Oh my gosh, it's it's over eighty million people. It's
like eighty six million people have borderline the high cholesterol.
So that's a that's a pretty good chunk of folks.
Seven percent of children and adolescents, so it's a big number.
I find that we're not looking at things like oxidized
(17:59):
heel e L, which is like the rate of rusting
of your ld L cholesterol which causes it the plaque,
or little life things like lipe of protein little A
or apo lipoprotein B. These are the fractions of cholesterol
that the higher those are, the more you're in trouble
(18:20):
because we need cholesterol. Cholesterol helps our cell membranes. It
helps us to build our hormones. It's you know, it's
a it's a building block. But not if we're making
bad building material.
Speaker 2 (18:33):
Yeah. Absolutely. And in the first the first part of
the show, I talked a little bit about cholesterol. So
in your words, Jim, I want to I want to
review with the listeners again, Uh, what is cholesterol? You know,
through throughout a lot of different terms there about the uh,
the L d l's oxidizing and things like that. So
so in your terms, what is cholesterol.
Speaker 6 (18:57):
Basically, it's of that molecule. It's found then in every
cell in your body, and it's and it's a necessary component.
So cholesterol is fat, fat like, and it is a
building block to create hormones and vitamin D and digestive
enzymes and protects the brain. So you know it's a
(19:22):
building block that's based on fat.
Speaker 2 (19:24):
M Yeah, so so important for a lot of those hormones,
the testosterone, estrogen, all those need cholesterol in the in
the body. So the we're talking about again, like the
main types of the main kinds of cholesterol. Can you
go into further detail on those, Jim.
Speaker 6 (19:45):
Sure. So, low density lipoproteins or LDL cholesterol is thought
to be what is like historically the bad cholesterol, but
we now know that the smaller the LDL the particle
size you can measure the size of it, the more
damaging it potentially can be. And the way I explain
(20:07):
that to people is, hey, think of the innerlining of
your ard or you're like a tennis net, and if
you're making L the L cholesterol the size of the softball,
you're not going to get too many throws paths through
the tennis net. Right, it's not going to penetrate and
go through the tennis net. But if you were making
BB sized cholesterol, how many bebes could you throw through
(20:32):
a tennis net? And that's and what shapes the size
of your lipids are things like the stress hormone cortisol,
is your insulin levels high? Are you insulin resistance or
do you have diabetes? Maybe environmental burden can do that. Obviously,
a lack of sexual moons, like if you're a stra
(20:54):
dial loone woman, if your testosteroso as a man. So
there's different reasons why it's stress, does it right, different
reasons why those lipids can go bad. Ahdls are supposed
to be good as long as they are big and fluffy.
If they're smaller, they don't protect us as much.

That's a good way to explain it. Jim, and we're

We've been talking about cholesterol today, Jim, and again, cholesterol
is very important and I want to want you to
tell the listeners why cholesterol matters.
Speaker 6 (23:24):
Well, I mean, the first thing is is that too
much cholesterol, especially if your lifestyle habits are poor, you're
eating the wrong food. You're spiking your blood glucose after
you eat, which is the single biggest thing that plaques
your argeries is the amount of inflammation that curs after
(23:44):
you spike your bood sugar. So you know, if your
cholesterol is too high, you're making bad actor cholesterol and
your lifestyle is not correcting for it. It basically penetrates
that arterial wall and then you're it's called infamal medial witness.
The inner linings of your eyeries get thicker and more stiff,
(24:05):
and now you develop aflscrosis. So you know, you lower
blood flow to your extremities, and of course, when you
can't get blood to your extremities and deliver oxygen to
your extremities, you get more damage to those tissues.
Speaker 2 (24:19):
And ultimately, if you're constricting that blood flow, it puts
you at a higher risk of heart attack and stroke.
And so, Jim, how do you know if you have
high cholesterol?
Speaker 6 (24:31):
Pretty straightforward. You just have to get blood tests, and
you want to do it fasting at least. I always
tell people ten hours, but at the minimum of an
eight hour fast, and then go get your cholestral tested down.
You can do that through a van puncher, like go
to a lab and get it drawn out of the
van your arm. They're also the fingersticks are pretty accurate,
(24:55):
especially if you go to a health there and they
have the immediate cholesterol readings. You could also do that
or it'll show you hdl LDL total cholesterol and triglycerides,
which is another risk factor when your triglycerides get elevated.
Speaker 2 (25:12):
Yeah, and Jim, so again, if we're talking just the
numbers of cholesterol, just a real baseline, quick baseline for
the listeners, where would you put each of those levels at.
Speaker 6 (25:27):
I mean two hundred for total cholesterol, LDL cholesterol should
be less than one thirty actually, and then LDLs if
you're a man greater than forty, for a woman greater
than fifty, I like them higher. And then obviously for triglycerides,
(25:49):
it's moving solidly towards one hundred or less being ideal.
Speaker 2 (25:55):
M Yeah, And that's an important pointing component in this
because it does trikel rides. It's another another type of
fat that that flows through the blood. And while they're
necessarily not basically part of that plaque that builds up
that high levels, they they help to uh, they basically
bring in those those LDLs and stick to the stick
(26:16):
of the walls a little bit more. And if you're
if you're talking about what what can you do if
you have high cholesterol, what can you do to lower that?
Speaker 6 (26:27):
Well, I mean age garlic extract has been my go
to for a long time. Obviously, changing your diet, exercising right,
like getting off sugar, really watching saturated SAT, especially if
you're a certain genotype. Saturated SAT's not good for you.
You know, getting adequate sleep, you know, you know, get
that exercise in regularly all good. But probably the most

proven compound is age garlic extract to otherwise known as kyolic.
Nine hundred publications. I mean they published six to eight
new studies a year. And the interesting thing onkyolic is
that in prouse blood pressure, they've had multiple randomized trials
showing that it lowers systolic blood pressure by about ten points,

lowers the diastolic pressure by about five points. So the
high number comes down, the low number comes down. What's
that mean? Your blood vessels are more reflectible. It reduces
oxidized LDL, So the damaging bad actor lipids like oxidized
LDL a fu life approachein b they go down when

been shown to regress or reverse vulnerable flat in humans.
Not in my study in humans it showed that. So
there isn't in my opinion, there isn't a single supplement
that does more for your kind of predio vascular health,

meaning you know the effect on the you know of
these bad actor lipids on your blood vessels plaquing all
of that.

Yeah, that's a that's an important to note. So it's

fats which are found in fried foods and processed foods

your LDL those low density lipoprotein level cholesterols. So a
good place to start with this is by switching out
maybe your butter or your margarine for a high quality
extra virgin olive oil or maybe even an avocado oil.

Then if you try making omega three is a part
of your diet, like a rich in salmon vegetable dishes,
serve those with your foods at least a couple times
a week. Also, data suggesting that if you use dietary
fiber and may lead to lower levels of your LDL cholesterols.

So it also reduces the risk of coronary heart disease.
So if we're talking about soluble fiber, which isn't necessarily
absorbed in the testin, but it binds to cholesterol and
removes it from the body. So if you're talking about
eating about five to ten grams of soluble fiber per
day can help lower those to LDL cholesterol levels by

as much as five to eleven points. So you're talking
about foods that are high in fiber include whole grains,
lean proteins, fruits, and vegetables. And you can get more
soluble fiber through supplementation as well where needed. And of course,
avoid sugar sugar sweetened beverages. They don't do your cholesterol
