Created as a way for patients and families to plan ahead and to cope with serious illness. Available in 23 languages and known as “the living will with a heart and soul” 1. The person you want to make decisions for you when you can’t. 2. The kind of medical treatment you want or don’t want. 3. How comfortable you want to be. 4. How you want people to treat you. 5.
