On débute par un essai randomisé marquant publié dans JAMA Pediatrics évaluant l’hypothermie thérapeutique chez les nouveau-nés prématurés entre 33 et 35 semaines. Résultat : une augmentation probable de la mortalité, sans bénéfice neurodéveloppemental. Ensuite, ils analysent une étude canadienne multicentrique (JAMA Network Open) qui démontre que la réponse précoce à l’INO chez les prématurés atteints d’HTAP est associée à une meilleure survie, surtout lorsque l’initiation est guidée par échographie. Une troisième étude (JAMA Network Open) met en évidence qu’une seule dose de corticoïdes anténatals, même administrée peu avant la naissance, améliore les issues cliniques chez les grands prématurés. Puis, Ben présente une étude qualitative (Archives of Disease in Childhood) sur le regret décisionnel chez les mères ayant accouché à la limite de viabilité, soulignant l’importance de la consultation prénatale. Enfin, un papier du Journal of Perinatology revient sur 15 ans de gestion de l’heure d’or dans les hernies diaphragmatiques congénitales au CHOP, avec des protocoles progressifs ayant permis une amélioration significative de la survie.
