L’épisode débute par une étude randomisée contrôlée comparant la dopamine à la norepinephrine dans le traitement du choc septique néonatal. Malgré une base physiopathologique en faveur de la norepinephrine, les résultats n’indiquent pas d’avantage clinique clair, soulignant le besoin de mieux définir les phénotypes hémodynamiques du choc chez le nouveau-né.
Ils poursuivent avec une étude observationnelle sur l’administration de dexaméthasone à faible dose selon le protocole DART. Les résultats mettent en évidence une amélioration du développement moteur à deux ans ainsi qu’une réduction des anomalies structurelles visibles à l’IRM cérébrale, en faveur d’un effet neuroprotecteur potentiel.
Enfin, ils discutent d’un essai clinique évaluant la darbopoïétine chez les grands prématurés. Bien que l’étude ne démontre pas d’effet significatif sur la cognition à deux ans, elle montre une réduction notable du nombre de transfusions nécessaires.
